Operations 1623 Harney Street Omaha, Nebraska 68102 Gentlemen: This refers to the inspection conducted by Messrs. P. H. Harrell, R. P. Mullikin, and T. Reis of this office during the period May 1-31, 1989,'of activities authorized by NRC Operating License DPR-40 for the Fort Calhoun Station, and to the discussion of our findings with you and other members of your staff at the conclusion of the inspection.- Areas examined during the inspection included review of previously identified items; licensee event report followup; operational safety verification; plant tours; monthly maintenance observations; monthly surveillance observations; security observations; radiological-protection observations; in-office review of periodic, special, and nonroutine event reports; followup of onsite events; and NRC Bulletin followup. Within these areas, the inspection consisted of selective examination of procedures and representative records, interviews with personnel, and observations by the NRC inspectors. The inspection findings are documented in the enclosed inspection report. During this inspection, it was found that certain of your activities were in violation of NRC requirements. Consequently, for Violation A you are required to respond, in writing, in accordance with the provisions of Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations. Your response should be based on the specifics contained in the Notice of Violation enclosed with this letter. For Violation B corrective action was i initiated, completed, and verified by the NRC inspector prior to the completion of this inspection; therefore no written response is necessary. Two unresolved item are identified in paragraphs 5 and 9 of the enclosed I inspection report. 1 We have also examined actions you have taken with regard to previously I identified inspection findings. The status of these items is identified in j paragraph 3 of the enclosed report.
