Combined NRC Region I Inspection Report Nos. 50-334/89-04 and
50-412/89-04
This refers to the routine resident safety inspection by Messrs. J. Beall and
S. Pindale at the Beaver Valley Power Station in Shippingport, Pennsylvania.
The reporting period covers the time from February 16 - March 31,1989.
The
inspection consisted of document reviews, interviews and observation of activ-
ities. The findings were summarized and discussed with you and members of your
staff at the conclusion of the inspection.
Based on the results of this inspection, it appeared that certain activities
regarding station procedures were not conducted in compliance with NRC require-
ments.
Please respond to this matter in accordance with the enclosed Appendix
A.
Also, several personnel errors and activity controls deficiencies contri-
buted to the March 22, 1989, safety injection. Please include in your response
the actions taken to address these additional concerns.
The response directed by this letter and the accompanying Notice are not sub-
Jcct to the clearance procedures of the Office of Management and Budget as
required by the Paperwork Reduction Act of 1980, PL 96-511.
