Gentlemen:

SUBJECT: MCGUIRE' AUGMENTED INSPECTION TEAM (AIT) REPORT

This refers to the special, onsite inspection conducted by the NRC Augmented

Inspection Team (AIT) during the period March 8-11, 1989. The inspection

included a review of the events related to the "B" steam generator tube failure

on - March 7, 1989, and subsequent recovery. At the conclusion of the

inspection, the findings 'were discussed with those members of nur staff

identified in the enclosed inspection report.

l ~ The AIT concluded that the tube failure did not result in a radiological

release to .the environment that exceeded regulatory limits or exceeded a

Technical Specification Safety Limit. The operating crew performed adequately

and they knew which procedure contained the actions or steps needed to

mitigate this event. However, the procedures were found to have significant

weaknesses which could result in unnecessary releases of radioactivity to the

environment should future steam generator tube rupture events occur. The

mitigative strategy which McGuire used for coping with this event deviated'

substantially from the Westinghouse Owner's Group Emergency Response

Guidelines. This was related to you in my letter of March 21, 1989,

concerning McGuire Nuclear Station Emergency Procedures, along with a request

for you to reevaluate these differences and document their justification for

our inspection by June 30, 1989. We are also concerned with operators lack of

confidence that certain systems would function as designed.

The enclosed copy of the AIT report identifies the areas examined during this

inspection. Within these areas, the inspection consisted of selective

examinations of procedures and representative records, interviews with

personnel, and observation of activities in progress.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and its enclosure

will be placed in the NRC Public Document Room.
