-Verification Function Inspection. conducted by S. A. Elrod from December 12, .
1988 to' January 26, 1989.
The inspection included a review of activities
authorized - forf your Sequoyah facility. At the conclusion of the inspection,
the' findings were; discussed with those members of your staff identified in the
enclosed. inspection report.

Areas examined _during the inspection are identified in the report. These area's
included a ' review of : plant operations; surveillance control, system ' outage
control, and work control processes; the corrective action prog' cam; maintenance
activities; the plant modification process; quality verification activities;
the independent qualified reviewer. process; and followup of events. Within
these areas, the inspection consisted of selective examinations of procedures
-and representative records, interviews with personnel, and observation of
activities in progress.
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The violation involved failure to take adequate corrective
action,
in the radioactive waste processing
area,
for a
previous
violation concerning control of system configurations and plant evolutions.
- The . violation, references to pertinent requirements, and elements to be
. included ~in your response are described in the enclosed Notice of Violation.
In addition,.the enclosed Inspection . Report identifies activities that appeared

to violate NRC requirements but are not cited; therefore, no response to these
j
items is required.

. Unresolved items were identified concerning trending within administrative
,'
' -
control programs,-inclusion of vendor manual torque requirements in maintenance
p
instructions., completion of workplan review and timely reporting of ASME Code
-
Section XI . repa. irs and replacements, and engineering evaluations of vendor
'
l
macuals. These matters will be pursued by the NRC staff during future inspec-
l
tions.
