NRC Bulletin No. 85-03, Motor Operated Valve Common Mode Failures During Plant Transients Due to Improper Switch Settings - Vermont Yankee Submittal of Final Report In accordance with the requirements of NRC Bulletin 85-03 [ Reference b)),  !

as supplemented [ Reference e)], Vermont Yankee herewith submits, as an enclosure to this letter, the requested final report.

The action items outlined in the Bulletin required Licensees to identify which valves met the Bulletin criteria, establish the design basis for.these valves, develop a switch setting program, stroke test each valve to determine its operability, and prepare procedures for determination and maintenance of switch settings. As detailed in the encksed report, Vermont Yankee's operabi-lity review, in response to Bulletin 85-03, for the condition of the subject Motor-Operated Valves (MOVs) was based on prior operating history in addition to j as-found thrust and limit switch settings. Where appropriate, new thrust and switch settings have been developed in accordance with our guidelines for selecting, setting, and maintaining switches for MOVs. These new settings increase the operability margin for the applicable MOVs. Valves have been j stroke tested, using valve analysis equipment, to verify correct switch set-tings, and procedures have been prepared and/or modified for the maintenance of these switch settings.

VERMONT YANKEE RESPONSE TO IE BUTU TIN 85-03 I. INULODUCTION As a result of several instances of on-demand, common mode failures of motor-operated valves due to improper switch settings, IE Bulletin 85-03, Reference No. 1, was sent to licensees requesting that they develop and-implement a program to ensure that switch settings on certain safety-related motor-operated valves are selected, set, and maintained correctly to accommodate the maximum Differential Pressures (DPs) expected in these valves during both normal and abnormal events within the design basis. In accordance with the requirements set forth in the subject bulletin, Vermont Yankee herein reports the results of our review and demonstration of the operational readiness of each motor-operated valve in the High Pressure Coolant Injection (HPCI) and Reactor Core Isolation Cooling (RCIC) systems that are required to be tested for operational readiness in accordance with 10CFR50.55a (g).

Vermont Yankee's program to ensure that valve operator switches are selected, set, and maintained properly is a three-fold program. First, the required stem thrust for each valve was calculated based on its highest calculated DP. Second, a motor-operated valve analysis system was applied to diagnose the valve's characteristics, set the motor operator to produce the required stem thrust, and verify limit switch settings. Third, a maintenance program was instituted to review each motor operator, to ensure correct and undamaged parts are installed, and to control and properly adjust motor operators to achieve their operational readiness. Procedures for operating the valve analysis equipment and setting limit switches have been written which reference stem thrust and limit switch settings requirements.

Vermont Yankee proposed to complete this three-fold program no later than the end of our scheduled Cycle 14 refueling outage. Within 60 days of completion of the program, Vermont Yankee committed to submit a written report providing verification of completion of the requested program.

As required by Bulletin Action Item (f), this report presents our I verification of completion for each of the individual action items, a summary of the findings as to valve operability prior to any adjustments resulting l

i The HPCI and RCIC Systems have been reviewed to identify the MOVs which are subject to the concerns. expressed in IEB'85-03. All MOVs identified in the Vermont Yankee response to the' Bulletin are shown in Table No. 1.

Table No. 1 lists by name and identification number the valves that were originally reported to be within the scope of IEB 85-03 in-Reference No. 2. . Valve V13-1 (trip throttle valve) has been excluded from the IEB 85-03 response since the motor operator is not. required, under any circumstances, to act'with an applied DP. The operator simply relatches the trip mechanism. Common stroke testing of this valve subjects it to the same conditions as any accident or mispositioning actf.on.

Valve V23-24 is a nonsafety MOV and, therefore, not subject to IEB 85-03 reporting requirements.

II.A IE Bulletin 85-03. Action Item a ]

For motor-operated valves in the high pressure coolant injection / core spray and emergency feedwater systems (RCIC for BWRs) that are required to be tested for operational readiness in accordance with 10CFR50.55a(g), develop l

and implement a program to ensure that valve operator switches are selected, set, and maintained properly. This should include the following components:

a. Review and document the design basis for the operation of each valve. This documentation should include the maximum DP expected l during both opening and closing the valve for both normal and I abnormal events to the extent that these valve operations and events are included in the existing, approved design basis. (i.e. ,

the design basis documented in pertinent licensee submittals such as FSAR analyses and full-approved operating and emergency I procedures, etc.). When determining the maximum DP, those single equipment failures and inadvertent equipment operations (such as inadvertent valve closures or openings) that are within the plant design basis should be assumed.

I Vermont Yankee Completion of Action IteDLB l

Vermont Yankee has completed review and documentation of the design basis for the operation of valves in the High Pressure Coolant Injection System (HPCI) and the Reactor Core Isolation Cooling System (RCIC). The results of this review are documented in Table No. 2.

The Vermont Yankee plant is a General Electric Boiling Water Reactor j

.(BWR) and has no High Pressure Core Spray System or Emergency Feedwater )

System. Therefore, Vermont Yankee has evaluated the high pressure coolant 7767R

injection function which is provided by our HPCI System and our RCIC System.

The HPCI System consists a of steam-driven pump, valves, and piping and is I used for accident conditions. The RCIC System consists of a steam-driven pump, valves, and piping and is used for abnormal plant conditions.

The HPCI and RCIC Systems have been reviewed to identify the MOVs which 1 are subject to the review expressed in IEB 85-03. All MOVs subject to a response in accordance with the bulletin are listed in Table No. 1.

Table No. 2 lists the valves by identification number and provides the design and maximum calculated accident DP requirements plus the required -

safety action for each valve.  ;

I The design DP is determined from plant drawings and equipment specifications. Prior to the 1989 outage, the design DP was used in j conjunction with a calculational method (Limitorque) to determine the minimum )

thrust to set the operator to produce.

In accordance with the Vermont Yankee response to IEB 85-03 (Reference No. 2), the accident DP for each valve has been calculated to encompass i

normal, accident, and mispositioning DPs.. Vermont Yankee used the "BWR Owners Group Report on the Operational Design Basis for Selected Safety-Related Motor-Operated Valves" NEDC-313322 (Reference Nos. 8 and 9) as a basis to determine maximum DPs. The NRC has accepted this report in Reference No. 10 as an acceptable methodology for selection of valves and their maximum expected DP.

.II.B.- IE' Bulletin 85-03. Action Item b l Using the results'from' Action Item (a) above, establish the correct switch settings. This shall include a program to review and' revise,-as' necessary,; the methods for selecting and setting all switches- (i.e.,- torque,-

torque bypass, position limit, and overload) for each valve operation ~(opening -g and closing).

If the license determines that a valve is inoperable, the licensee shall.also'make an appropriate justification for continued operation in accordance with the applicable' technical specification.

Vermont Yankee's switch setting program established for IEB 85-03 valves followed the guidelines provided below.

Switch Setting Guidelines

1. Selecting
a. Open Limit Switch I

The open limit switch will be set such that the valve either does not or only lightly comes in contact with the back seat due to its momentum and inertia. The switch may initially be set at 95% of full open and adjusted, if necessary, by use of the information in the valve signatures. .

b. Closed Limit Switch A closed limit switch is not used on any Vermont Yankee bulletin valves,
c. Open Bypass Limit Switch The limit switch is set at approximately 20-25% of valve travel beyond the hammer blow of the operator. 20% to 25% of valve travel is more than sufficient to ensure that the effects of DP l on opening have reduced to less than the open torque switch setting.  !
d. Close Bypass Limit Switch The closed bypass limit switch is set such that it covers at least that part of the stroke during which intermittent forces such as hammer blow occur. It is set at approximately 95% open 4 and adjusted if necessary according to valve signatures to l l

ensure valve closure on torque. )

e. Close Torque Switch
1. Minimum Setting (a) Calculate the DP at accident conditions (inadvertent valve actuation encompassed) see Table No.2.

(b) Use this accident DP in a statistical data base (MOVATS) to get a comparable thrust. This thrust is the minimum acceptable thrust the valve can be set to. .

m ii. Target Setting The minimum thrust. setting has a 10% margin added to'it t to achieve a target thrust. This target thrust is used in the field as the nominal thrust setting.

iii. Maximum Setting The maximum setting is based on the limitations of the operator.

f. Open Torque Switch l

i. The open torque switch settings will be set as determined in II.B.1.e above.
g. Thermal Overloads  !
i. Thermal overloads for all IEB 85-03 valves are sized such that the ability of the valve to complete its safety function is not jeopardized by an undersized thermal overload device.
2. Setting
a. The setting of limit switches is done using an approved procedure (Reference No. 5) and an approved drawing (Reference No. 6). Use of valve signature traces will be used, when available, to help set the switches in accordance with l Reference No. 6.
b. The setting of torque switches is performed using an approved l procedure, Reference No. 5, and valve signatures using ,

Reference No. 7, to accomplish the switch setting that yields l

1 the specified thrust values in Reference No. 6.

c. Thermal' overload protection devices were' reviewed and

determined to be the correct ' size as determined in Il-B-1.g. .

3. Maintaining Switch Settings
a. All limit switch settings, once completed, are not' subject to drift and; therefore, require checking.or resetting only after-the following maintenance activities:
1. Removal'of operator from valve.

ii. Removal of limit switch.

iii. Removal of stem nut iv. Any repair requiring removal of any gearing during some' pact of the maintenance evolution.

b. Torque switch settings will be checked, as a minimum..by verification of, thrust once every three outages for refueling.
c. Thermal overload protection devices are.specified by controlled drawings.

II.C IE Bulletin 85-03. Action Item c Individual valve settings shall be changed, as appropriate, to those established in Item'(b), above. Whether the valve setting is changed or not, the valve will be demonstrated to be operable by testing the valve at the maximum DP determined in Item (a), above, with the exception that. testing motor-operated valves under conditions simulating a break in the line containing the valve is not required. Otherwise, justification should be' provided for any cases where testing with this' maximum DP cannot practicably be performed. This justification should include the alternative to maximum DP-testing which will be used to verify the correct settings.

Note: This bulletin is not intended to establish a requirement for valve testing for the condition simulating a break in the line containing the valve. However, to the extent that such valve operation is relied upon in the design basis, a break in the line containing the valve should be considered in the analyses prescribed in Items (a) and (b) above. The resulting switch settings for pipe break conditions should be verified, to the extent practical, by the same methods that would be used to verify other settings (if any) that are not. tested at the maximum DP.

Each valve shall be stroke tested, to the extent practical, to verify that the settings defined in' Item (b), above, have been properly implemented i even if testing with DP cannot be performed.

1 Vermont Yankee completion of Action Item c J

The Vermont Yankee response, Reference No. 2, to NRC IE Bulletin 85-03 stated that full design DP testing at Vermont Yankee was not viable. All of the valves within the scope of the Bulletin for Vermont Yankee HPCI and RCIC Systems were re,"c u~ to determine if an acceptable DP test could be performed. The HPCI and RCIC Systems are steam powered systems making DP I testing during a shutdown virtually impossible. Therefore, DP testing must be done during power operations when these systems are required to be operationally ready. Because of the status of these systems during operation, only two bulletin valves were within the qualifications necessary to allow DP testing, V13-131 and V23-14. These valves are normally stroked during system i monthly surveillance and are capable of being used as emergency steam isolation to the turbines.

NOTE: Two other safety-related globe valves, not in the HPCI or RCIC Systems, were also DP tested. The results of these DP tests were similar to the l results of V13-131, so only results of testing the bulletin globe valve (V13-131) will be reported here.

Special test procedures were developed to allow and control the use of these valves as steam isolation valves and, therefore, obtain a DP (998 psid)

I which is near the calculated accident DP of 1080 psid. During the DP test, valve analysis equipment was installed to record thrust and switch movement.

The original method for DP testing, as presented in the IEB 85-03 Response Reference No. 2, was to calculate the available DP at test conditions and reset the torque switch appropriately. If the valve stroked successfully, the Vermont Yankee methodology for setting switches was considered acceptable. This approach was discarded in favor of obtaining more performance information about the MOV while stroking against the applied DP.

The selected valves are physically different. V13-131 is a 3" globe- J valve, and V23-14 is a 10" gate vrive. Neither valve has special design features, and they are typical of other valves throughout the plant.

Table No. 4 presents the results of this testing compared against the calculational method (Limitorque) for determining required thrusts. The conclusion is that the calculational method is marginally acceptable for globe valves and that the calculational method for gate valves should include some additional conservative measures, such as using a higher " design" DP or increasing the " valve factor."

Table No. 5 presents a comparison of the extrapolated thrust for the accident DP to the calculated thrust (Limitorque method) using design DP and the data base thrust ("MOVATS") at accident DP. This shows that the data base thrusts are more conservative than either the extrapolated thrusts associated with the accident DP or the calculational method (using the more conservative design DP). However, it also shows that pre-1989 outage settings using design DP calculations were adequate to demonstrate operability by comparison to the extrapolated test thrust requirement for accident DP.

Based on the general conclusions reached by the industry and confirmed by Vermont Yankee DP testing, Vermont Yankee has set all bulletin MOVs above a minimum thrust, correlated to the calculated accident DP, using the "MOVATS" statistical data base (see Table No. 3). Using a statistical data base provides the necessary conservatism to verify that Vermont Yankee MOVs that cannot be DP tested have properly selected thrust settings.

III. VALVE EVALUATION Vermont Yankee has completed the evaluation of switch settings calenlated in response to Action Item (b) for the HPCI and RCIC valves subject to this bulletin. Vermont Yankee's determinations of the valve as found condition was based on information generated from the Action Items, the operation history of the valve, and the control wiring diagrams. The data resulting from the bulletin action items provided the necessary information for making our determinations.

A swnmary of our evaluation of the "as found" condition of each valve is contained on each valve's summary sheet (Attachment 2).

Each valve subject to the bulletin requirements has been mechanically refurbished by cleaning, greasing, parts replacement,etc., and with the exception of V13-15 upgraded to 4-train limit switch assemblies.

The review of each valve was conducted using the guidance for setpoints presented in Section II.B. " Vermont Yankee Completion of Action Item (b)."

Although not specifically required by the bulletin, other evaluations for operability were also conducted for Vermont Yankee bulletin valves. A review for sufficient motor torque was conducted to ensure each motor is sufficiently sized to allow operation of the valve in its accident condition.

Using the required ruotor torque and the applicable motor curves for each direct currect (de) motor, a review for adequate current has been performed. This review was actually performed for Information Notice 89-11 "DC Cable Sizing Verification," but does apply in some respects to the IEB 85-03 issue. This review confirms that all de powered IEB 85-03 valves have sufficient available current to operate under cccident conditions.

IV. OREN_IIEMS Motor-operated Valve V13-15 is not fully in accordance with the switch settings guidelines described in Section II.B. Two open items exist:

1. The original two-train limit switch assembly has.not been replaced l

with a four-train assembly, which is necessary for meeting the  ;

liinit switch settings guidelines.

2. The spring pack has not been replaced with a heavy-duty spring pack which is necessary to. meet the torque switch settings guidelines. f i

V13-15 is located within the drywell in very close proximity to i stru::tural steel. The four-train assembly requires a larger limit switch cover, and sufficient clearance to the structural steel is not available. In order to allow enough time to properly analyze structural steel modifications or valve / piping changes, V13-15 has been upgraded by eliminating the open torque switch bypass and open torque switch. With the twc-train limit ewitch, the open torque switch bypass cannot be set at 20-25% witbout a b o affecting position indication limit switches on the same rotor. V13-15 opens on limit  ;

at approximately 95% valve open, but does not have mid-strohe valve protection normally provided by the open torque switch. In order to provide valve protection during tho opening stroke, motor thermal overloads are provided, i

sized such that motor damage will be prevented. )

1 During the 1989 outage, a heavy duty spring pack for V13-15 was l unavailable. The lighter spring pack was retained and the close torque switch l was bypassed until just prior to seating. Operator momentum carries the valve disk into the seat to provide proper sealing.

V13-15 is considered by Vermont Yankee to he acceptable in accordance with the requirements of IEB B5-03. Elimination of the open torque switch and essentially closing on limit are accepted methods of valve control which are used throughout the industry. During a future outage, a four-train limit switch and a heavy duty spring pack will be installed to allow switch settings in accordance with Section II.B.

V. CONCLUSIDN All Vermont Yankee valves presently within the scope of IE Bulletin 85-03 have been reviewed to the requirements of IEB 85-03 and are considered operable within those requirements.-
