'Page 3 of 14 REVIEW, APPROVAL AND REVISION OF TEST INSTRUCTIONS / PROCEDURES 1.0 PURPOSE The purpose of this procedure is to establish the requirements and responsibilities for review, approval and revision of startup test 4
instructions and procedures.
I 2.0 APPLICABILITY This procedure is applicable to all Prerequisite Test Instructions, 3
and Preoperational Acceptance, and Special Performance Test procedures l dL used during the startup of CPSES Units 1 and 2.-
3.0 DEFINITIONS Prerequisite Tests - Tests performed on individual components to verify complete installation, operability, cleanliness, i
calibration, etc. Also referred to as checkout, or initial checkout.
1 Acceptance Tests - Tests conducted on non-safety related station systems to demonstrate their ability to satisfactorily perform their design function.
Preoperational Tests -
Tests conducted prior to fuel loading to demonstrate the capability of components, systems 1
or structures to meet safety-related performance requirements.

J Special Performance Tests -
Tests conducted to demonstrate component / system performance, collect operating data or to control special testing activities.
Results of Special Performance Tests will not be used to comply with test commitments in the Final Safety Analysis l
Report.
Deviation - Departure from a previously approved methcd, sequence, wording or standard.
l Revision - A new, amended, improved or up-to-date version.

4.0 PROCEDURE 4.1 Prerequisite Test Instructions 4.1.1 Review and Approval Each Prerequisite Test Instruction shall be reviewed by the Lead Startup Engineer, the TUGC0 QA Startup/ Turnover

a manuals will be distributed in accordance with the distribution list contained in each manual.. Manual receipt will be documented on Attachment D and returned l
to the Manager, CPSES Startup.

Note Special Prerequisite Test Instructions will be distri-buted as a separate manual.
4.1.2.2 The latest revision of all Prerequisite Test Instructions will be distributed to each manual holder.

4.1.2.3 Manual holders are responsible for ensuring that their i
manuals are up-to-date by inserting and reading.new or revised instructions and destroying all superseded f
material or marking superseded material as " SUPERSEDED" or " VOID" to prevent use, q
l 4.1.2.4 Upon receipt of new or revised procedures, the manual

holder shall sign and date.the receipt acknowledgement space provided on the' transmittal letter,-see Attachment C, and return the' letter to the Manager, CPSES Startup.
4.1.2.5 The initial issue of each approved test instruction will be designated as Revision 0.
Subsequent revisions will be numbered sequentially using Arabic numberals.
4.1.2.6 Each test instruction revision will be reviewed and 3
approved in accordance with paragraph 4.1.1.

4.2.3.2 Approved test procedures will be revised when deemed necessary by the Lead Startup Engineer.
4.2.3.3 When any portion of an approved procedure is revised.
the entire procedure will be issued as a-new revision.
4.2.3.4 Each procedure revision will be reviewed and approved in accordance with paragraph 4.2.1.3 and 4.2.1.4 Note If a revision is significant, as judged by the Manager, Nuclear Operations, further review may be initiated as specified by paragraph 4.2.1.1 prior to re-vision approval.
4.2.3.5 All copies of approved test procedures, except the original and the copy used for conduct of the test, will be stamped "For Information Only."
4.2.3.6 The test procedure copy to be used during conduct of the test will be stamped " Official Test copy."

4.3.3.4 Each procedure revision will be reviewed by the JTG members specified in paragraph 4.3.1.3 and approved in accordance with paragraph 4.3.1.4.
4.3.3.5 All copies of approved test procedures, except the original and the copy used for conduct of the test, vill be stamped "For Information Only."

4.4 Special Performance Test Procedure 4.4.1 Review and Approval As a minimum Special Performance test procedures will be reviewed by an individual qualified as Level IV in accordance with CP-SAP-19 and approved by the Lead Startup Engineer.

Test Objective (a). Determine that the stated objective is sufficient to describe what the procedure is to accomplish.
Section 2.0:
Acceptance Criteria (a) Determine that.the specified criteria are consistent.with-project documents such as purchase ' and/or design :
specifications. Final Safety Analysis Report.
(b) Determine that sufficient criteria are specifiedlto support judgement of acceptability of the system and/or component per,formance.

References (a) Determine that the identified references provide sufficient information fer. procedure review..
(b) Determine that referenced material revision correspond to latest published revision.

(d) Determine that all procedure attachments are properly identified.
(e) Determine that provisions are included on all attached data sheets for recording the name of the person that recorded the test data and date.
(f) Ensure that the use and restoration of off-normal conditions of system components are included. (e.g., jumpers, lifted leads, etc.).
(g) Determine that the procedure steps specified to be witnessed are adequate to ensure quality of the test.
(h) Ensure that the procedure has utilized to the extent practical normal and emergency station operation procedures.
