FERMI:2' STATUS REPORT FOR THE: PERIOD AUGUST 17-28, 1987 Enclosed is the Fermi 2 bi-weekly status report from the NRC resident office

at Fermi 2. Two NRC resident, inspectors monitored licensee activities at the plant during this report period.

Status reports of this type are intended to provide NRC management and the public with an overview of. plant activities and NRC inspection activities.

Subsequent monthly inspection reports will address many of these topics in more detail.

FERMI STATUS REPORT FOR THE PERIOD AUGUST 17-28, 1987

1. Plant Status As of 0800 on August 28, 1987, Fermi 2 was in cold shutdown.
2. Facility Operations Sumary The licensee placed the reactor in cold shutdown on August 2, 1987, to

, repair a-leaking feedwater check valve. Additional maintenance activities have been added to the outage scope to improve the material condition of the p'lant. The licensee projects that all maintenance  ;

activities will be completed in early September. Upon completion of j these activities a management meeting between the licensee and NRC will '

take place to determine whether the NRC concurs with the Detroit Edison Company assessment of readiness for plant restart.

3. Item (s) of Special Interest Annual Emergency Exercise - On August 17, 1987, the licensee conducted  ;

their annual emergency exercise. The exercise was considered l successful with some deficiencies noted.

Masonry Block Wall Design - On August 17, 1987, the licensee identified two block walls in the control room which could fail in a design basis earthquake. Other walls were identified not to meet the safety factors stated in the FSAR. All walls will be repaired to their FSAR safety factor prior to facility restart.

These structural problems were i an outgrowth of a Region III inspection which identified calculational errors for the walls.

Backup Manual SCRAM Breaker (BMSB) - On August 7, 1987, a BMSB failed to actuate during switching of Reactor Protection System bus A from normal to alternate power. The licensee completed investigation / troubleshooting activities associated with the BMSB. The investigation concluded the  !

root cause of the failure to be poor workmanship during installation of the breaker.

I Raychem Splices - On August 21, 1987, the licensee determined that  ;

Raychem heat shrink insulation for some electrical connections were i improperly installed on two High Pressure Coolant Injection (HPCI) system valves. The connections were a part of a sample of connections inspected by the licensee. As a result of the improper installations, the environmental qualification of the connections is suspect. The inspection was initiated as a result of recent industry experience with deficient splices. The licensee is expanding the Raychem inspections to all connections where heat shrink is known to have been installed for harsh environment considerations.  !

f, Operation of the Reactor Water Cleanup '(RWCU) system - On. August 21, 1987, the licensee determined that the operation of the RWCU system without.the heat exchangers in service, the bypass mode, was an , j unacceptable. operational mode. . Motor Operated Valves - The licensee is continuing to. rewire the open e circuit on motor operated valves. Originally, the lhensee wired numerous.M0Vs to open on torque. When inquiries were made of the valve r9 j manufacturers as to whether this was an acceptable practice, all but Powell stated that this configuration was taken into account when l designing the valve. Therefore, all but the Powell valves age being rewired to open on limit prior to plant startup. The Powell valves will be rewired by the end of the first refuelingfoutage. The vendor-inquiries were made as an outgrowth of a Region III inspection of ,

IEB 85-01. The licensee started conducting MOVATS testing on 9 valves in '

the HPCI and Reactor Core Isolation Cooling (RCIC) systems on August 15, 1987. No results of the testing are available at this time.

Unplanned ESF Actuation - On August 27, 1987, RPS bus A lost power when j' the applicable motor generator output breaker tripped on undervoltage.

This-condition resulted in a half SCRAM, closure of all the Main Steam ,

Isolation valves (MSIV) and loss of shutdown cooling due to some containment isolation valve automatic closures. The MSIV closures occurred due to ^!

surveillance testing in division 2 concurrent with the power loss in division 1. Shutdown cooling was restored with no significant increase in ,{

reactor temperature, i l

Management Changes - At the beginning of August the Engineering Vice President was appointed plant manager until another plant manager is selected. The Operations Vice Pru ident was appointed Engineering Vice ,

President. The previous plant manager was aopointed Director of Nuclear 1 Engineering. The previous Director of Nuclear Engineering was transferred out of the nuclear organization. All personnel appear to be functioning in their new responsibilities.

4. Change to Monthly Status Reporting This is the first biweekly status report for Fermi.

The report will continue to be biweekly with the next report due on September 24, 1987.

5. NRC Staff Activities During the Period A Region III inspection was conducted on the licensee MOVATS activities during the week of August 17th. Another Region 111 inspection was conducted on the >

l MSIV logic during the week of August 24th. A Region 111 inspection was initiated during this reporting period concerning Torus structural support modifications. An emergency exercise observation was conducted during the week of. August 17th. The resident staff followed outage activities and consisted of two inspectors as identified below:

Senior Resident Inspector - Walt Rogers Resident. Inspector - Mike Parker On August 24, 1987, the NRC conducted a SALP/50.54(f) public meeting at the lccal community college.with the Regional Administrator presiding.

The SALP ratings were:

Category 1 - Emergency Preparedness, Startup Testing Category 3 - Operations, Surveillance, Training / Qualifications Not Rated - Fire Protection Category 2 - For the remaining areas The Regional Administrator emphasized that these ratings reflected below average performance.
