NOTICE OF VIOLATION Philadelphia Electric. Company

-NRC inspectors' reviewed the circumstances associated with'a violation of at technical specification limiting condition for operation which was identified
-by the licensee and reported to the NRC.
In accordance with~the " General
. Statement of Policy and Procedure for NRC Enforcement, Actions,"'10 CFR Part 2, AppendixC'(1987),theviolationissetforth.below:
. Technical Specification Limiting Condition for Operation (LCO) 3.11.A.5 requires that whenever the control room emergency ventilation system is required to be operable in accordanca with Technical Specif1.

! safe, or emergency filtration of the control room ventilation intake air must be initiated. ' Technical Specification LC0 3.11.A.1 requires that the control room emergency ventilation system be operable whenever secondary containment integrity is required.
Contrary to the above, prior to May 29, 1987,'during periods when secondary containment integrity was required,.both control room' intake air'_ radiation monitors were inoperable and emergency filtration of the control room' air was not initiated. The radiation monitors were inoperable in that the monitors _were receiving a sample of emergency ventilation suction rather than normal ventilation suction because the sensing lines to the sample valve had been reversed.
This is a Severity Level IV Violation (Supplement I).
Pursuant to the provision of 10 CFR 2.201, Philadelphia Electric Company.is hereby required to submit a written statement or explanation to the V.S.
Nuclear Regulatory Commission, ~ ATTN: Document Control Desk, Washington, DC 20555 with a copy to the Regional Administrator, Region I, and a copy to the NRC Senior Resident Inspector within 30 days of the date of the letter trans-mittinglthis Notice.
This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation:
(1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
