Subject: NOTICE OF VIOLATION AND ENFORCEMENT CONFERENC6 REPORT
(NRC Inspection Nos. 50-277/87-17; 50-278/87-17)
This refers to the NRC_ resident safety inspection conducted on June 1 to
July 17, 1987 at the Peach Bottom Atomic Power Station, Units'2 and 3, Delta,.
Pennsylvania. The inspection report was sent to you on July 31, 1987. During
'the inspection,. inspectors reviewed the circumstances associated with a viola-
tion of a technical specification limiting condition for operation (LCO) which
was identified by your staff and reported to the NRC.
On August 13, 1987, an
enforcement conference was conducted with you and members of your staff to
discuss the violation, its cause, and your corrective actions. The enforcement
conference report is enclosed.
The violation, which is described in the enclosed Notice, involved the in-
operability of the control room intake radiation monitoring system.
The system
appeared to have been inoperable since initial installation sometime prior to
1974 in that the sample lines from the normal ventilation duct and the emergen-
cy ventilation duct to the sampling valve were reversed. As a result, when
certain high radiation conditions existed, the two radiation monitors would not
receive a representative sample of the air supply and therefore, would not
transfer air supply from the normal ventilation system to the emergency
ventilation system, and isolate these systems in the time frame intended by
the original design.
The NRC recognizes that the safety significance of this condition was minimized
by the' fact that (1) subsequent calculations indicate that the delay in

trans,fer of the air supply to the emergency ventilation system would.have
resulted in only a minimal increase in the radiation exposure to control room
personnel, and (2) a continuous area radiation monitor also existed in the
control room, and emergency procedures required operators to periodically
observe this monitor and take action to manually transfer suction to the
emergency ventilation system in the event of a high radiation condition.
Nonetheless, the NRC is concerned that the sensing lines for these radiation

monitors were apparently not properly installed during initial construction,
and post-installation testing of the systems was not sufficient to detect this-
error. These events demonstrate the importance-of proper installation and
-suitable testing whenever work is performed at the facility.

Therefore, although the violation described in the enclosed Notice has been
classified at Severity Level IV in accordance with the " General Statement of.
Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C
(Enforcement Policy) (1987), any similar violation in th future may result in-

additional enforcement action.

..You are required to' respond to the enclosed Notice and, in preparing your.
response, you should follow the instructions specified in the Notice.
In your
respense, you'should document the' actions taken or planned to prevent recur-
rence. Specifically,'your response should reflect (1) the changes.to_your
analysis that were described in the Licensee Event Report which you discussed

during the enforcement conference, and (2) the results of your investigation
'
to establish that no similar conditions exist i_n other systems important to-
safety for which the sens. ors have not be tested' or demonstrated to function
properly by an actual initiation. After reviewing your response to the Notice,
including your proposed corrective actions and the results of future inspec-
tions, the NRC will determine whether further enforcement action is necessary
to ensure compliance with NRC regulatory requirements.
In accordance with 10 CFR 2.790, a copy of this letter and its enclosures will
be placed in the NRC Public Document Room.
The responses directed by this letter and the enclosed Notice are not subject
to the clearance procedures of the Office of Management and Budget, otherwise
required by the Paperwork Reduction Act of 1980, Pub. L. No.96-511.
