K. A. Connaughton and G. F. O'Dwyer, of this office on June 2 through

August 12, 1987, of activities at Perry Nuclear Power Plant, Unit 1,
authorized by NRC Facility Operating License No. NPF-58 and to the
discussion of our findings with M. D. Lyster and others of your staff

at the conclusion of the inspection.

The enclosed copy of our inspection report identifies areas examined
during the inspection. Within these areas, the inspection consisted
of a selective examination of procedures and representative records,
observations, and interviews with personnel.
During this inspection, certain of your activities appeared to be in
violation with NRC requirements, as specified in the enclosed Notice.

A written response is required.

Under the " General Statement of Policy and Procedure for NRC Enforcement
Actions," 10 CFR Part 2, Appendix C, Supplement I.C.1, Item 1 of the
enclosed notice could be categorized as a Severity Level III.
However,
considering the time frame over which the violation occurred and the fact
that the fire service water piping remained filled and pressurized well
in excess of calculated containment peak accident pressure when the
associated manual containment isolation valves were open, we have
classified this violation at Severity Level IV.
As discussed in Paragraph 6. of the enclosed inspection report, reviews
of corrective action implementation for reportable events were conducted
during this inspection. These reviews identified a number of instances
where implementation of corrective actions specified in Licensee Event
Reports had been deferred for considerable periods of time, though
restraints upon implementation were not evident. We are concerned that

the indiscriminate deferral of corrective action implementation may result
in repetitive reportable events such as those identified -in Item 4 of
the enclosed notice. Please' include in your response to Item 4 that
steps you have or will'be taking to ensure.that corrective actions for
reportable. events will be implemented in a timely manner.
In accordance with 10 CFR 2.790.of the Commission's regulations, a copy
of this letter, the enclosures, and your response to this letter will
be placed in the NRC Public Document Room.
The responses directed by this letter and the accompanying Notice are
not subject to the clearance' procedures of..the Office of Management and
Budget ;as required by the Paperwork Reduction Act of 1980, PL 96-511..
We will gladly discuss any questions you have concerning this inspection.
