i During an NRC inspection conducted during the period July 1-31, 1987, violations of the NRC requirements were identified. The violations involved i
failure to perform and document a safety evaluation for a facility change and failure to properly lock a manual valve.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), t'ne violations are listed below:
A.
According to 10 CFR 50.59(a)(1), a licensee may make changes to the facility as described in the safety analysis report without prior Commission approval unless the proposed change involves a change in the Technical Specifications or an unreviewed safety question.
Paragraph (b)(1) requires the licensee to maintain records of such changes, including a written safety evaluation which provides the bases for the determination that the change does not involve an unreviewed safety question. Section 5.2.7.1.1 of the ANO-2 safety analysis report l
states that the containment atmosphere monitoring system (CAMS) sample is returned to containment atmosphere.
Contrary to the above, on July 23, 1987, the NRC inspector found that the samples drawn by CAMS Units 2RITS-8231 and 2RITS-8271 were being discharged via hoses to the auxiliary building ventilation system, but no safety evaluation for this change had been performed, i

Unit 2 Technical Specification 6.8.1 requires that written procedures be l
established, implemented and maintained covering the activities
(
recommended in Appendix "A" of Regulatory Guide 1.33, Revision 2, February 1978. Appendix "A," Item 3.d requires procedures for the emergency core cooling system.
Plant Operating Procedure 2102.01, " Plant Preheatup and Precritical Checklist," has been established in accordance with this Technical Specification requirement.
Attachment E, " Category E Valve Position Verification," of Plant Operating Procedure 2102.01 requires that Manual Valve 251-118 be locked closed when i
the swing High Pressure Safety Injection (HPSI) Pump 2P89C, is aligned to l
supply the "A" high pressure safety injection header. Manual l
Valve 2SI-11B is the discharge cross connect isolation valve between HPSI J
t Pumps 2P89B and 2P89C.

i Contrary to the above, on July 22, 1987, with the plant operating at 100 percent power, and HPSI Pump 2P89C aligned to supply the "A" HPSI header, the NRC inspector found that Manual Valve 2SI-11B was closed as required, but not locked. The lock was attached to a broken link in the chain. This permitted removal of the lock and ' chain without use of a key.
This is a Severity. Level IV violation.

Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, D.C. 20555 with a copy to the Regional Administrator, Region IV, and a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice. This reply, should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation:
(1) the reason for the violations if admitted, (2) the corrective steps that have been taken and the resdits achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
If an adequate reply is not received within the time specified in this Notice, an l
order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be j
taken. Where good cause is shown, consideration will be given to extending the i
response time.
