This refers to the routine safety inspection conducted by Messrs. T. M. Tongue,
l
W. J. Kropp, T. E. Taylor, and N. Choules of this office on June 21 through
l
August 1, 1987, of activities at the Braidwood Station, Units 1 and 2,
l
authorized by NRC Operating License No. NPF-72 and Construction Permit
!
No. CPPR-133 and to the discussion of our findings with Mr. E. E. Fitzpatrick
and others of your staff at the conclusion of the inspection.
The enclosed copy of our inspection report identifies areas examined during
the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representat.ive records, observations, and
interviews with personnel.
During this inspection, certain of your activities appeared to be in violation
i
of NRC requirements, as specified in the enclosed Notice. A written response
is required.
In accordance with the NRC Enforcement Policy, escalated enforcement action
was considered for Item 1 of the Notice of Violation.
Review of this matter
revealed that the safety significance of the event was not as serious as if it
had occurred under other conditions such as a history of higher power levels
and, upon discovery, prompt effective corrective measures were taken.
We are
concerned about sone of the circumstances that contributed to the event, such
as personnel not believing instrumentation, e.g. , alarms clearing without
explanation, and the need to pursue conflicting information,
e.g., the cleared
annunciator and the observations of personnel involved.
In addition to your
response to the Nctice of Violation, please describe your actions that you
have taken or will take regarding our concerns.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosures, and your response to this letter will be

placed in the NRC's Public Document Room.
The responses directed by this letter and the accompanying Notice are not
subject to the clearance procedures of the Office of Management and Budget as
required by the Paperwork Reduction Act of 1980, PL 96-511.
