NORTH ANNA UNIT 1 (NA-1) PRIMARY TO SECONDARY LEAK EVENT, JULY 15, 1987 Prior to 0630 hours0.00729 days <br />0.175 hours <br />0.00104 weeks <br />2.39715e-4 months <br /> on July 15,1987, NA-1 was operating at 100% power. At 0630, the Main Steam Line-C radiation monitor registered Hi-H1 alann and the CR0 noted PZR level and pressure decreasing and therefore increased charging l

flow to the RCS. The unit was manually tripped at 0635 and at approximately the same time a Lo-Lo pressure safety injection signal was received (PZR pressure less than 1700 psig and PZR level less than 5%). At 0639 a Notification of Unusual Event was declared and at 0650 feedwater flow to steam generator (SG) C was isolated but SG-C level was identified to be increasing indicating SG Tube Rupture /or Break. At 0654 an Alert was declared and at 0705 SI was terminated. At 0710 emergency procedures were initiated for post-SGTR cooldown using backfill. The Technical Support Center was activated at 0757 and the Local Emergency Operations Facility activated at 0915. The unit entered Mode 4 (Hot Shutdown) at 1108 and at 1218 the RHR system was placed in service l

and the unit entered Mode 5 (Cold Shutdown) at 1330 hours0.0154 days <br />0.369 hours <br />0.0022 weeks <br />5.06065e-4 months <br /> with the event terminated at 1331 hours0.0154 days <br />0.37 hours <br />0.0022 weeks <br />5.064455e-4 months <br />.

It is noted that no automatic actuation of primary or secondary safety relief I

valves occurred. Total radioactivity release was 0.265 Curies dose equivalent-Xenon-133. This is less than 1% of TS limits (instantaneous release rate).

A tube leakage rate of 30-50 gpm has been quoted but '*;is range should be held suspect until the leakage rate can be more accurately calculated and SG-C l

inspected upon cooldown (several days).

It is also noted that dur(ng the recent NA-1 refueling outage (April 17 to June 30, 1987) all three SGs were 100% inspected. During this outage, SG-C had 117 tubes plugged bringing the total plugging for SG-C to 270 tubes or 8% of total.

i 2-At the present time the plant is stable and a team of regional inspectors are on site as well as the VEPC0 recovery response team.

Finally, offsite environmental monitoring teams have detected no increase in radioactivity l

above nomal background level.

SUBJECT:

STEAM GENERATOR TUBE LEAK North Anna Unit 1 experienced a steam generator tube leak this morning while operating at 100% power.

A notification of Unusual Event was made at 6:39 a.m and an Alert declared at 3

6:54 a.m.

The plant was manually tripped at 6:35 a.m. and Safety Injection was automatically initiated at 6:36 a.m.

Safety Injection was terminated at 7:05 a.m.

The NRC resident l

inspector was in the control room during the event.

The Regional and Headquarters Emergency l

Response Centers were activated in the standby mode.

A team of regional inspectors was dispatched to the site. The plant condition is stable Virginia Power reported an estimated release of radioactive material,to the atmosphere of 1

less than 1.3 percent of the Technical Specification release limit. ~The release path has j

been isolated.

Offsite environmental monitoring teams detected: no increase in radioactivity above normal backaround.

i The Commonwealth of Virginia, the EPA and FEMA have been notified.

Virginia Power has issued a press release and held a press conference at 9:30 a.m in Richmond Virginia.

The plant reached cold shutdown this afternoon at 1:31 (EDT), and the utility has j

downgraded from an Alert Status to the recovery mode.

The NRC has deactivated its emergency response centers in Washington and Atlanta but continues to closely monitor events at the site.

i A team of NRC inspectors from the Atlanta regional office is onsite, along with the j

resident inspectors from Surry and a public affairs officer from Washington.

1 Media interest has been brisk, and the licensee has held several news conferences.

The licensee briefed 38 reporters at the site, and the NRC will provide information to interested media representatives.

1 FRC inspectors will remain at the site to follow up on licensee actions and to assess the secuence of events.

There is no estimate available at this time as to how long the plant may be shut down for repairs.

The Commonwealth of Virginia has been kept informed and has dispatched a representative to the site.

SUBJECT:

AIT FORMED AT NORTH ANNA This PN updates PNO-II-87-47 and II-87-48, issued July 15, 1987.

1 The regional team which was sent to the site on July 15, in response to a steam generator tube leak has been expanded into an Augmented Inspection Team. The AIT was formed on l

4 July 16 after the licensee calculated a possible leak rate of as much as 600 gallons per minute.

The Final Safety Analysis Report (FSAR) for this unit assumes that one tube could leak as much as 785 gallons per minute without affecting the safe shutdown of the reactor.

The licensee plans to visually inspect the steam generator this weekend for a more.

precise reading on what has occurred and is reconstructing circumstances associated with the event on the plant simulator.

Media interest continues.

The licensee conducted plant site tours for the media and for l

local areas officials yesterday and will continue to do so today.

Region II continues to keep the Commonwealth of Virginia informed.
