With respect to Item A.2, additional
information was presented which shows that A.2.c, A.2.d, A.2.e,
A.2.f, and A.3 complied with procedures CP-CPM-6.3 and
QI-QAP-11.1-39; therefore, these portions of Violation A are
withdrawn.
