(NRC INSPECTION REPORT NOS. 50-413/87-30 AND 50-414/87-30)
This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by
P.
K. VanDoorn and M. S. Lesser from August 26 - September 25, 1987.
The
inspection included a review of activities authcrized for your Catawba
facility. At the conclusion of the inspection, the findings were discussed
with those members of your staff identified in the inspection report.
The
report documenting this inspection was sent to you on October 14, 1987.
NRC
concerns relative to the inspection findings were also discussed with you in an
Enforcement Conference held on November 6, 1987. A summary of this Enforcement
Conference is enclosed.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The inspection findings indicate that certain activities appeared to violate
NRC requirements.
The violation, references to pertinent requirements, and
elements to be included in your response are described in the enclosed Notice
of Violation.
Beause the Auxiiiary Feedwater System would have performed its intended
function the inoperability of the auxiliary feedwater pump was found to be a
Severity Level IV violation.
However, during the Enforcement Conference your
recent history of instrument isolations was discussed, including our concerns
of their similar natures.
Therefore, in your response to this letter please
address your actions to assure operability of instrumentation.
Your response
should describe your corrective actions to detect mispositioned instrument
valves and to prevent future inadvertent mispositioning of instrument valves;
and a description of your program to ensure surveillance / calibration of
safety-related instruments.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,
Title 10, Code of Federal Regulations, a copy of this letter and its enclosures
will be placed in the NRC Public Document Room.
