NRC INSPECTION REPORT 50-285/98-09

Thank you for your letter of July 15,1998, in response to our June 25,1998, letter and

Notice of Violation concerning the failure to have an adequate procedure for performing an

activity that affected quality. Specifically, Procedure OP-3A, " Plant Shutdown," Revision 19,

directed operators to use pressurizer steam space temperature indication to monitor plant

cooldown when, in fact, pressurizer liquid space temperature should have been utilized. We

have reviewed your reply and find it responsive to the concerns raised in our Notice of Violation.

We will review the implementation of your corrective actions during a future inspection to

determine that full compliance has been achieved and will be maintained.

REPLY.T0 A NOTICE OF VIOLATION

During an NRC inspection conducted on April 12 through May 23, 1998, one

violation of NRC requirements was identified.

In accordance with the " General

' Statement of Policy and Procedure for NRC Enforcement Actions," NUREG 1600,

the. violation is listed below:

-10'CFR'Part 50, Appendix B, Criterion V, states, in part, that

activities affecting quality shall be prescribed by documented

instructions,~ procedures, or drawings of_a type appropriate to the

circumstances and shall be accomplished in accordance with these

' instructions, procedures, or drawings.

Contrary to the above on April 2, 1998, it was determined that the

licensee did not have an adequate procedure for performing an activity

thet affected quality. Specifically, Procedure OP 3A, " Plant Shutdown,"

Revision 19 directed operators to use pressurizer ' steam space

temperature indication to monitor plant.cooldown_when, in fact.

pressurizer. liquid space temperature should have been utilized. This

resulted in operations personnel allowing reactor coolant system

pressure to decrease below the minimum required pressure for maintaining

net positive suction head for the operating reactor coolant pump.

This is a Severity Level IV violation (Supplement 1)(50 285/9809 01).

Reason for the Violation

The reason for this violation was inadequate review of a procedure change.

The procedure change that incorporated using the steam space temperature

indication had been marked " stet" indicating that the change from liquid space

to steam space temperature should not be incorporated. The change was

incorporated and not discovered during subsequent reviews. Prior to this

. event ~ the practice was to only review those portions of the procedure that

individuals knew were being completed.

Corrective Steps Taken and Results Achieved

Following this event. Operating Procedures and many of the Operating

l

Instructions were reviewed prior to use during the 1998 refueling outage.

l

This review was completed by a former Assistant Plant Manager-0perations, and

a Senior Nuclear Design Engineer, both previous Senior Reactor Operator

License holders at Fort Calhoun Station. Additionally, a side-by-side, draft-

to-final review of th; entire procedure is currently being conducted on

procedure changes.

Corrective Steps That Will Be Taken To Avoid Further Violations

Procedure OP-3A will be revised to remove reference to the pressurizer staam

space temperature by August 1. 1998. This procedure is also being revised to

incorporate Emergency Core Cooling System equipment operability requirements

during shutdown modes as conmitted to in LER-1998-006 (Reference 3).

Conducting a complete. thorough, administrative and technical review of

procedure changes will be re-emphasized to typing reviewers. Qualified

Reviewers, and Responsible Department Heads by July 31, 1998.

Standing Order G-30. " Procedure Changes and Generation." is being rewritten to

improve the overall procedure change process and will be revised by August 31.

1998.

The date when full compliance will be achieved

Fort Calhoun Station will be in full compliance by August 1.1998.
