i ENCLOSURE 1 NOTICE OF VIOLATION Florida Power Corporation Docket No. 50-302 Crystal River Unit 3 License No. DPR-72 During the Nuclear Regulatory Commission (NRC) inspection conducted on October 20-21, 1987, a violation of NRC requirements was identified.
The violation i
involved a failure to maintain and implement written procedures for Emergency Plan implementation.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violation is ' fisted below:
10 CFR 50.54(q) requires that a licensee follow and maintain in effect emergency plans which meet the standards of 10 CFR 50.47(b) and 10 CFR Part 50, Appendix E.
10 CFR 50.47(b)(4) requires in part that the licensee use a standard emergency classification and action level scheme.
Technical Specification 6.8.1.e requires that the licensee establish, implement and maintain written procedures for Emergency Plan implementation.
Emergency Plan Implementing Procedure (EPIP) EM-202, " Duties of the Emergency Coordinator," provides instructions and guidelines to be utilized by the Emergency Coordinator during initiation of the Radiological Emergency Response Plan.
Section 5.g of Enclosure 1 to EPIP EM-202 indicates an Alert is the appropriate classification for the "all alarms lost" condition.
Section 6.0, " Records and Notifications," of EPIP EM-202 requires the retention of significant information for accident reconstruction.
Contrary to the above, the implementing procedures were not properly maintained and implemented in that:

Paragraph 5.g of Table 8.1 in the Emergency Plan states that the condition of "all alarms lost" with plant transient or greater than or equal to 15 minutes would be classified as a Sitel rea Emergency whereas Paragraph 5.g of Enclosure 1 to Emergency Plan Implementing Procedure (EPIP) EM 202 states that the condition of "all alarms lost" for greater than or equal to 15 minutes with plant transient would be classified as a Site Area Emergency.

An "all alarms lost," condition occurred on October 14, 1987, and was not classified as an Alert.

The annotated copy of EM-202 that was used during the Alert declaration on October 16, 1987, with the "all alarms lost" condition was not retained.

Pursuant to the provisions of 10 CFR 2.201, Florida Power Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice a written statement or explanation in reply including:
(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further s
violations, and (5) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
