i NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-374 k

As a result of the inspection conducted on September 22 through November 3, 1987, and in accordance with 10 CFR Part 2, Appendix C - General Statement of Policy and Procedure for NRC Enforcement Actions (1985), the following violation was identified:

Technical Specification 6.2.A.1 requires that written procedures be prepared, approved and adhered to, including Administrative Procedures.

LaSalle Administrative Procedure LAP 100-30, " Independent Verification," step F.3.e.1 states, "When lifting leads, always use the wiring diagrams and wire color indication if it is available." LAP 240-6, " Temporary System Changes,"

paragraph E.5 states, "All Temporary System Changes will be installed using the wiring diagrams and all terminal wires vill be verified to be correct according to the color code of the wires per the wiring diagram and physical installation at the terminal board."

Contrary to the above, on October 17, 1987, electrical maintenance personnel failed to adhere to LAP 100-30 and LAP 240-6 and used electrical schematics Residual Heat Removal (RHR)g diagrams to remove a failed relay in the Unit 2 instead of electrical wirin system. A relay in the Reactor Water Cleanup System (RWCU), which was in series with the RHR relay was then actuated on loss of power when leads were lifted to the RHR relay, causing an RWCU isolation.

This is a Severity Level IV violation (Supplement I). (374/87029-02)

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
