NRC Inspection
This refers to the inspection conducted by Mr. Kent M. Prendergast and Mrs.
Mary E. Solberg from chis office on June 1-5, 12-and 16, 1987 of activities
authorized by NRC License Number DPR-54, and to the discussion of our findings

held by Mr. Prendergast with Mr. Carl Andognini and ott'er members of. your

. staff at the conclusion of the inspection.
Areas examined during this inspection are described in the enclosed inspection
report. Within these areas, the inspection consisted of selective examination
Based on the results of this inspection, it appears that certain of your
activities were not conducted in full compliance with NRC regulations, as set
forth in the enclosed Notice of Violation.
Your response.to this notice is to be submitted in accordance with the
provisions of 10'CFR 2.201 as stated in the enclosed Notice of Violation.
The enclosed Notice of Violation (NOV) concerns repetitive deficiencies
identified during Health Physics / Medical Drills. We are particularly
concerned with the implications of the first aid drill in as far as the NRC
issued a NOV (dated October 28, 1986) to SMUD for failure-to conduct first aid
training as required by the SMUD Emergency Plan.
SMUD replied to this NOV by
letters dated December 1, 1986 (JEW 86-923) and December 31, 1986 (JEW
86-982).
It appears that the actions described in these letters have not been
totally effective in assuring properly trained personnel are available to -
-
provide first aid treatment.
The examples of the enclosed NOV indicate that

SMUD management may not be effectively identifying the root cause of all-
i
problem areas, instituting appropriate corrective actions and providing
follow-up to assure completion.
The fact that actions to assure an effective
i
emergency preparedness program cross many organizational lines may be a
(
contributing factor. We urge SMUD to carefully examine the lessons that may
be learned from the last medical and health physics drills. We further. urge
that SMUD~ carefully examine its first aid treatment program to assure adequate
capabilities are available.
In accordance with 10 CFR 2.790(a), a copy of this letter and enclosures will
be placed in the NRC Public Document Room.
