APPENDIX A NOTICE OF VIOLATION Maine Yankee Atomic Power Company Docket No 50-309 Maine Yankee Nuclear Generating Station License No. OPR-36 As a result of the inspection conducted on June 11 - July 17,1987, and in accord-ance with the Enforcement Policy (10 CFR 2, Appendix C), the following violation was identified:
Technical Specification 5.12.2 requires, in part, that locked doors shall be provided to high radiation areas in which the radiation intensity is at such levels that a major portion of the body could receive in any one hour a dose in excess of 1000 mrem to prevent unauthorized access to those areas.
Contrary to the above, on June 17 and June 25, 1987 a door located in the upper level of the Spray Building, which controls access to the lower levels of the building where whole body radiation levels exceed 1000 mr/hr, was not in a locked condition.
The above instances of failure to adequately control access to a locked high radiation area constitutes a Severity Level IV Violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, Maine Yankee Atomic Power Company is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement of explanation in reply, including: (1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Where good cause is shown, considera-tion will be given to extending this response time.
