INCOMPLETE LICENSING EVENT REPORTS

We have found many Millstone 3 Licensing Event Reports (LERs) containing statements in the text or abstracts such as "all ESF equipment operated as designed." The intent of the LER rule is to get complete reporting, that is, the LER writer is to describe all aspects of the event.

(NUREG-1022, Supplement No. 2, specifically states that statements such as "all systems responded as designed" are not sufficient.) Such statements should not be made without naming all of the systems that actuated or isolated. Such statements do not address safety system response adequately, and are not acceptable as complete reporting. They require a knowledge of the detail design of tne particular plant to understand the event.

We request that you name the systems actuated or isolated for all 1986 and 1987 Millstone LERs which contain statements such as "all ESF equipment operated as designed." Examples of such LERs include 86-001

-003, -012,

-015, -019, -021, -028, -032, -035, -049,87-001, -002, -008, -020, -021,

-026.

This request is not for supplemental information beyond that required for the contents of any LER, but merely a request for complete reporting.

The Licensee Event Report rule (10 CFR 50.73 Section (b) Contents) states that "The Licensee Event Report shall contain:...(2)(i) "A clear, specific, narrative description of what occurred so that knowledgeable readers conversant with the design of commercial nuclear power plants, but not familiar with the l

details of a particular plant, can understand the complete event.

(ii) The narrative description must include...(K) Automatically and manually initiated safety system responses."
