i The Boston Edison Company is committed to achieving excellence in operation of the Pilgrim Nuclear Power-Station and has undertaken substantial plant material improvements in two major areas:

)

o Enhancing the inherent safety of the plant by completing  !

certain plant modifications; and o Upgrading the material condition of the physical plant and invoking high standards for maintaining the plant in top material condition.

This action plan addresses the second of these areas. Plan-development and implementation are discussed below.

RESTART actions are aimed at completing scheduled outage work, O reducine the meintenence deck 1ee, correctine chronic meintenence-related problems, and making interim changes pending development and implementation of longer term improvements. In general, items not designated as RESTART actions address long term, continuing improvements with the goal of achieving and sustaining the highest standards of maintenance performance throughout the i

normal plant operating cycle.

The criteria used to select RESTART actions are as follows: l i

1. The iten identifies a specific deficiency or problem which  !

reasonably can be expected to lead to the.in' operability or a ,

limiting condition, in accordance with the Technical Specifications, of a system required for start-up or for normal operation.

2. The item identifies a programmatic deficiency ' which BECo O- management assesses is likely to cause a specific deficiency meeting the definition in No. 1 above.

3. The item identifies a specific deficiency resulting in (

failure to comply with NRC regulations and no variance has been approved by NRC. _

4. The item represents a management commitment by BECo to improve the inherent safety of the plant to levels which exceed the requirements of the Technical Specifications and the NRC.

5. The item is an external commitment (NRC, INPO, ANI, etc.) to i complete prior to restart.
6. The item contributes to an unacceptably high back?.og of minor corrective maintenance actions which could have an  !

1 adverse cumulative effect on powcr generating systems.  ; {

7. The item is an action resulting f2om' ucM assessment which senior BECo management has assesscd essential to support timely and correct completion of RFO #7.
8. The item is a minor improvement which BECo manage:aent has assessed can readily be completed in the short term without diverting resources from critical path work.

This action plan is a living program which will continuously track the evolving improvement of the Pilgrim Station material  ;

condition. As long term actions are developed and refined the f required actions and closure requirements will be revised and f made more precise. As resources become available from the press of completing work associated with Restart, dates and responsibilities for long term (continuing) actions will be {

Q PLAN DEVELOPMENT AND IMPLEMENTATION v

'i nis action plan was developed through a rigorous process of 1 inspection and assessment that included the following:

1. First, a clear understanding of current conditions was established by thoroughly inspecting and evaluating the overall maintenance program using industry professionals with proven capability for managing maintenance and evaluating complex industrial (nuclear) operations.
2. Second, the conditions which should exist at Pilgrim Station were established after careful evaluation with successful managers within the industry and by reviewing successful maintenance operations at other similar plants.
3. Third, a plan with clear responsibility, accountability, and closure criteria for actions was established through extensive evaluation by BECo managers and consultation with successful industry managers.
4. Fourth, the plan was developed and is being implemented under the close direction of senior BECo managers.

CONDITION ASSESSMEHI.

The initial assessment focused on work identification and planning, schedule preparation, work assignment, work progress, i workforce supervision and discipline, workmanship practices, material condition, and management control. This involved:

1. Interviewing upper level managers to determine applicable policies and management methods. J
2. Reviewing procedures established for implementing policies.
3. Analyzing working documents, schedules, and other

communications to evaluate the degree of adherence in following established pr6cedures.

4. Observing work in progress and interviewing working level personnel to determine actual practice.
5. Inspecting plant spaces and systems. l I

I VERIFICATION.  !

O- The initial assessment of existing conditions was verified by ]

independent reviews conducted by a Maintenance Review Team from i the Institute of Nuclear Power Operations (INPO) and by two' teams of experienced consultants from within the nuclear power industry. These reviews were mutually consistent and reinforced )

the resolve of BECo senior managers to implement decisive. action to improve performance. .

DESIRED CONDITIONS.

To enable appropriate selection of actions, Desired Conditions were identified during the maintenance review. These condition j statements focus attention on the results to be achieved and, when compared to existing conditions, on the changes required to company and employee practices.

CORRECTIVE ACTIONS.

For each identified problem area, recommended RESTART and long h term corrective actions were proposed by the teams identifying or addition, verifying existing and desired conditions. In cognizant BECo managers evaluated the identified problems from j their own perspectives and provided independent recommendations.

From these various inputs, BEco managers distilled a final plan oi- action 'with clear definition of the results to be achieved.

For each RESTART action, a responsible manager is assigned and an f action completion date established. For each long term action, a start date is established, CLOSURE CRITERIA. ,

Specific criteria for closure of each action item are included.  !

l Implementation progress will be monitored by senior managers 3 weekly for RESTART actions and nonthly for long term actions.

The objective is to insure that responsibilities are clearly defined and that staff resources provided are commensur-

Numerous administrative demands are being placed on Boston Edison maintenance managers and supervisors that impact the time needed to properly supervise subordinates. Some key positions are being filled by contractors who are not assigned the same approval authority granted Boston . Edison supervisors. As a result, j available Boston Edison supervisors are traditionally burdened j with administrative work that should be completed by contractor j supervisors.

1 DESIRED CONDITION:

Unnecessary administrative duties of line management should be minimized. Contractors filling Boston Edison supervisory positions should be granted administrative approval authority commensurate wit'h the positions they are filling.

Priority "A" maintenance requests are assigned for immediate accomplishment, automatically.taking precedence over other work, with no management review made of material or labor resources-available, or the ' impact on work ' in progress. (It is not uncommon for up to 30% of the. manpower in a trade to be held in reserve to accomplish "A" priority work.) Backlog work items are

Material reaching the end of its shelf life should be analyzed to extend its lifetime or be disposed of and reordered.
