! SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-338/87-30 AND 50-339/87-30)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by

B. K. Revsin at the North Anna Facility on August 25-27, 1987. The inspection

included a review of events associated with an exposure in excess of regulatory

limits of the skin of a worker's whole body. The report documenting this

inspection was sent to you by letter dated September 9, 1987. As a result of

this inspection, failures to comply with NRC regulatory requirements were

identified, and accordingly, NRC concerns relative to the inspection findings

were discussed during an Enforcement Conference held on September 24, 1987. A

letter summarizing this Enforcement Conference was sent to you on October 1,1987.

The violations described in the enclosed Notice of Violation involved a failure

to maintain occupational exposures to the skin below regulatory limits, failure

to perform adequate radiological surveys, and failure to adhere to Radiation

Work Permit procedures involving protective clothing. The event that resulted

in these violations involved a contract health physics technician who had been

performing work in Unit I containment on the evening of August 19, 1987. Upon

exit from the Radiation Control Area, the technician was found to be contaminated.

Subsequent isotopic analysis showed that the contamination consisted of a

single microscopic particle of pure cobalt-60 with a total activity of 1.6 micro-

curies. The dose to the skin from the particle was determined to be 23.6 rem

based upon an estimated 3.25 hour2.893519e-4 days <br />0.00694 hours <br />4.133598e-5 weeks <br />9.5125e-6 months <br /> exposure time. When added to previous skin

exposure for the quarter, this localized skin dose increased the total dose to

the skin of the whole body for the third quarter of 1937 to 23.8 rem.

Violation A described in the enclosed Notice of Violation, which involves an

exposure to the skin in excess of regulatory limits, has been categorized as a

Severity Level IV violation. As indicated in Supplement IV of the NRC Enforce-

ment Policy, overexposure of this type are normally classified as Severity

Level III violations. However, after careful consideration of the factors

involved in this specific instance, we have classified this violation as a

Severity Level IV since the health implications of this exposure hre very small

due to the small area of skin that was exposed. The remaining violations have

also been categorized as Severity Level IV violations.

ensure compliance with NRC regulatory requirements.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2, '

Title 10, Code of Federal Regulations, a cnpy of this letter and its enclosure

will be placed in the NRC Public Document Room.

The response directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980, PL No.96-511.
