This preliminary notification constitutes EARLY notice of an eventoof POSSIBLE safety or public interest significance. The information p_ r resented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

Subj ect: HYDRAULIC OIL FIRE AND SUBSEQUEh"I INADVERTENT ACTUATION OF ECCS At 9:35 p.m. on December 29, 1976, a 1/4" hydraulic oil line on #12 {

Main Steam Isolation Valve (MSIV) severed during attempts to open the valve with the reactor critical in the startup mode (Mode II). Oil c was sprayed on hot piping in the immediate area, which resulted in ..

smoldering insulation. The insulation sevenminutesusingdrychemicalandCO{irewasextinguishedinabout extinguisher. After the fire was+ extinguished, the reactor was shutdown. Pressure and temperature '

were reduced to 1500 psig and 3900F,.respectively (Mode III) while repairs to the MSIV were made.

Due to operator error, the MSIV inadvertently opened after the hydraulic system was repaired, reducing the pressure in #12 Steam Generator suf-ficiently to actuate ECCS. The recovery from inadvertent safety injec-tion and cleanup after the fire are proceeding routinely.

i Media interest is expected because of previous inadvertent actuations of ECCS and close attention being given to problems at nuclear facili-ties. The State of New Jersey has been informed. A press release is not planned at this time.

NRC, Region I, (Philadelphia) receired notification of this occurrence by telephone from the station superintendent at 10:00 a.m. on December 30, 1976.
