APPENDIX A NOTICE OF VIOLATION Public Service of New Hampshire Docket No. 50-443 Seabrook Unit 1 License No. NPF-56 During an NRC inspection conducted on May 5 - July 6, 1987, a violation of NRC requirements was identified.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions", 10 CFR Part 2, Appendix C (Enforcement Policy 1986), the violation is listed below:
10 CFR 50.59 requires that written safety evaluations be performed which pro-vide the bases for determining that changes, tests or experiments do not in-volve an unreviewed safety question.
The Seabrook Station FSAR, Section 7.2.3.4 specifies the measures to be taken to ensure that safety reviews are conducted and that responsible station personnel are aware of the changes which may affect their duties.
Chapter 11210 of the NHY Programs and Proce-dures manual requires that safety evaluations be conducted and documented for all design changes and temporary modifications.
Procedure MA4.3, Temporary Modifications, specifies that safety evaluations must be performed on all temporary modifications.
Contrary to the above, on May 21, 1987 the service water system was identified to be in a configuration in which partial service water flow could be diverted to the screen wash system via a temporary jumper hose.
A safety evaluation had not been performed to ensure that this modification did not involve an unreviewed safety question.
Furthermore, responsible station staff members were unaware of this modification which had been initiated by the Operations Department.
Specifically, the Technical Support Department who are respons-l ible for overall work control on this system had no knowledge of the jumper or its potential effect on system operations.
This failure to perform the required safety evaluation resulted in a safety related system operating in an unreviewed configuration.
This is a Severity Level IV Violation (Supplement I).
You are not required to submit a response to this letter based on the corrective actions and actions taken to avoid recurrence which you implemented during this report period.
