i ENO.0SURE 1 NOTICE OF VIOLATION- j Florida Power Corporation Docket No. 50-302 Crystal River License No. DRP-72 ,

t During the Nuclear . Regulatory Commission (NRC) inspection conducted on September 28-October 2,1987, a violation of NRC requirements was identified.

The violation involved failure to maintain adequate records to properly implement the approved respiratory protection program. In accordance with the

" General Statement of Policy and orocedure for NRC Enforcement Actions,"

10 CFR Part 2, Appendix C (1987), the violation is listed below:  :

10 CFR 20.103(c)(2) requires that the licensee maintain and implement a respiratory protection program that includes determination by a physician j prior to initial use of respirators that the individual user is physically i able to use respiratory equipment. .

Technical Specification 6.8.1 states that written procedures be (

established, implemented and maintained covering the applicable procedures j recommended in Appendix A of Regulatory Guide 1.33, November 1972.  !

Appendix A of Regulatory Guide 1.33, states that respiratory protection l activities should be covered by written procedures.

Plant Procedure RSP-101, Basic Radiological Safety Information and ,

Instructions for Radiation Workers, Section 2.4.4, Rev. 7, November 18, i 1986, states that qualification in CR-3's Respiratory Protection Program must be obtained prior to using any respiratory protective equipment for the purpose of preventing the inhalation or ingestion of radioactive  :

Plant Procedure RSP-500, Health Physics Respiratory Qualification Program, Section 2.4.16, Rev. O, January 17, 1987, states that each functional area should forward lists of individual's completion requirements as often as necessary to keep the Nuclear Operator Training Information System (NOTIS) data base current.

l Contrary to the above, on September 29, 1987, the licensee failed to l maintain adequate records to implement the approved respiratory protection program in that a technician who did not have appropriate medical q qualification documentation in the NOTIS system was issued and wore a '

full-face respirator while cor. ducting health physics job activities in the radiation control area. )

Pursuant to the provisions o'f 10 CFR 2.201, Florida Power Cor; oration is'hereby required .to submit to this Office within 30 days of the date of the letter

' transmitting this Notice a written statement or. explanation in reply including:

.(1) admission or denial of the violation, (2) the reason for the violation if ,

admitted,. (3) the corrective ' steps which have been taken and the results achieved, (4) the corrective steps which will 'be taken to avoid further violations, and (5),the date when full compliance will be achieved. LWhere good-cause is.;shown, consideration will be given to extending the tesponse time.
