SUMMARY

Scope: This routine, unannounced inspection was in the areas of followup on

licensee action on previous inspection findings and the snubber surveillance

program.

2. Exit Interview

The inspection scope and findings were summarized on July 2,1987, with

those persons indicated in paragraph I above. The inspectors described

the areas inspected and discussed in detail the inspection findings. No

dissenting comments were received from the licensee. The following new l

item was identified during this inspection.

- Violation Item 321, 366/87-14-01, Failure to Have Written Procedures

to Cover Inspection of Mechanical Snubbers After Severe Dynamic

Event, paragraph Sa,

The licensee did not identify as proprietary any of the materials provided

to or reviewed by the inspector during this inspection.

3. Licensee Action on Previous Enforcement Matters

(0 pen) Unresolved Item 321, 366/84-05-02, Timely Closecut of Design Change

Requests. Review of the status of Design Change Requests (DCR) during .

previous inspections disclosed that the DCRs were not being closed out in

a timely fashion after completion of the actual physical modifications to

the plant. Example of items that need to be resolved prior to DCR

closeout are: verification by knowledgeable engineers that as-built

,

drawings have been completed, that all affected procedures have been

I revised, that NCRs and field deviation reports have been resolved, and

that revisions to various other documents (e.g. equipment indexes) have

been completed. As a result of the . inspector's concerns, the licensee j

formed a task force to address DCR closeout. The task force, named the l

DCR closeout group, reviewed 486 DCRs. Of this number, 292 were closed or

voided 152 were determined to need additional implementation, and the i

status of 42 could not be determined. The task force was disbanded in  !

June 1985. The inspectors discussed the current status of DCR closeout

'

with licensee engineers. These discussions disclosed that a total of 1043 1

DCRs were currently open. Of this number, 452 were ready to be closed,

'

) 182 were to be voided, 358 were in various stage of implementation, while

the status of the remaining 51 was not indicated. Some of the open DCRs i

date back to 1977, 1978. The licensee indicated that additional emphasis I

will be made to reduce the backlog of DCRs now that the Unit 1 outage work l '

is completed. This item will remain open pending further review by NRC

Region II.

4. Unresolved Items

Unresolved items were not identified during this inspection.

5. Snubber Surveillance Program, Units 1 and 2 (70370) ]

The inspectors examined procedures and quality records related to the I

snubber surveillance program and inspected snubbers on safety-related ,

piping systems. Acceptance criteria examined by the inspectors appear in l

Unit 1 Technical Specification 3/4.6.L and Unit 2 Technical Specification

3/4.7.4.

a. Review of Snubber Surveillance Procedures

The inspectors examined the following procedures whicn control

snubber surveillance activities:

(1) Procedure numbers 52SV-SUV-001-IS and 52SV-SUV-001-25, Hydraulic

Shock and Sway Arrestor Inspection and Functional Test.

(2) Procedure numbers 52IT-MME-002-1S and 521T-MME-002-25,

Inspection and Testing Pacific Scientific Mechanical Snubber. l

(3) Procedure number 52GM-MME-025-01, Removed of Pipe Hangers, l

l Restraints and Supports.

l

l Review of the above procedure and discussions with responsible

l engineers disclosed that the tables listing safety-related snubbers

'

in procedures were currently being updated and revised. Some of

these revisions were due to addition of new safety-related snubbers  ;

to systems and expansion of boundaries of safety-related systems

which resulted in inclusion of additional snubbers under the TS

l

surveillance program. The licensee identified the discrepancies in

the snubber listings on Deficiency Card 1-87-381 during the

Spring 1987 refueling outage.

Review of procedures 52IT-MME-002-IS and 2S disclosed the following

violation:

Units 1 and 2 TS 6.8.1 requires that written procedures be establish- j

ed covering activities referenced in Appendix A of Regulatory Guide

(RG) 1.33. Paragraph 8.b of RG 1.33 Appendix A requires that proce-

dures be written and implemented for each surveillance test listed in

the TSs. Contrary to this requirement, procedures were not written

l' to cover inspection of mechanical snubbers required by Unit 1 TS ]

4.6.L.2.(3)(a) and (b) and Unit 2 TS 4.7.4.(3)(a) and (b) to deter-

'

mine if there had been a severe dynamic event and verify the snubbers

were not frozen up. This violation was identified to the licensee as l

b. Inspection of Snubbers i

The inspectors performed a visual inspection of the .' following

snubbers:

(1) Snubber numbers 1E11 RHR 193*, IE11 RHR 213, 216 and 217 on the j

Unit 1 RHR system

(2) Snubber numbers 2E11 RHR 268, 269 A/B and 220 on the Unit 2 RHR

system

(3) Snubber number IE21 CSH-21 as the Unit 1 core spray system

hydraulic type. _l

During the above inspection, the inspectors verified that the

snubbers were not damaged, that attachment to the supporting

structure (hanger) and piping was secure, and for hydraulic snubbers

that the reservoir levels acceptable. One hydraulic snubber

examined, number 2G11 RHR 269B, had a reservoir level more than one

increment below full capacity when examined in accordance with

procedure 52SV-SUV-001-IS. Per the procedure this snubber was '

considered to be low in fluid and required replacement. The licensee

'

installed a new snubber in this location on July 2,1987, under

maintenance work order 2-87-2446. The licensee will perform a

functional test on the removed snubber to determine if it was

operable,

'

c. Review of Quality Records l

The inspectors examined quality records documenting visual inspection

of Unit 1 inaccessible hydraulic snubbers performed during April-May

1987. The inspectors also examined the results of functional tests

performed on mechanical snubber numbers 1 831-F3-H 800, 1EH-RHRH-724,

1 E 11 RHRH-728, 1 N11-MSH-50A, and 1 N11-MSH-43.  !

Within the area examined, no-deviations were identified.
