The concerned individual raised a specific concern that the method of installing amphenol connectors on actuators of standby diesel

]

generators is inadequate and could contribute to unreliable l

operation of the diesel generator.

l 1

Inadequate installation of the amphenol _ connectors has been substantiated by several reports. Maintenance Request (MR)

  1. A560178 (App. A, 5.a; 08/27/85) was issued as a result of Potential Reportable Occurrence (PRO) No. 1-85-272 (App. A, 5.b; 08/27/85) to investigate the problem that occurred when diesel generator (D/G) 2B-B failed to respond correctly during testing per l

surveillance instruction 51-7.1.

The investigation undertaken in i

PRO No. 1-85-272 determined that the electrical connector between

.the control panels and the hydraulic actuator was loose on engine l

2B-B, thus rendering the engine's hydraulic drive motor inoperable.

The problem was acknowledged by TVA. As immediate corrective action, the connector was tightened to ensure proper electrical C3 (l.J

' connection between the control panels and the hydraulic actuator.

v Subsequently, when the diesels were started, the motor for the hydraulic actuator operated properly.

l PRO No. 1-85-288 (App. A, 5.c; 09/04/85) reported that all of the f

1 connectors were visually checked for the same type of installation. All of the connectors were installed by the vendor 1

with a thread mismatch between the amphenol connector and the conduit fitting.

TVA indicated this mismatch resulted from the vendor _ using National Pipe Threads on the conduit fitting with machine threads on the connector. Several of the connectors were j

loose to the touch, but 28-B was the most critical.

PRO No.

i 1-85-288 also found that force threading of the conduit coupling 1

threads on the amphenol connectors by the vendor during assembly caused the amphenol connector thread failure and that the threads on the back side of the female connectors were stripped and broken.

A 09/06/85 telecopy from the D/G manufacturer Power Systems (PS), a

_i Morrison-Knudson Division, acknowledged the defect as reportable l

under 10CFR21 and indicated that the NRC would be accordingly notified. This notification was given to NRC-on 09/06/85 (App. A, i

5.r).

PS also indicated that in their opinion "ths defect does not render the diesel generator inoperative but recommend that surveillance be immediately initiated and the corrective action (install adapter) be done promptly."

The fi,st step undertaken by TVA to correct this problem was to install a Raychem ziplock sleeve (App. A, 5.j through m) over the connector and conduit coupling for additional support between the connector and the conduit coupling thus ensuring proper electrical

connection and mechanical strength. This corrective action was completed on 09/07/85. As a permanent fix to prevent recurrence,.

new wiring harnesses were ordered on 09/11/85 from the vendor

]

(added to contract #453168) to replace the original defective

(

harness assemblies. MRs (App. A, 5.d through g) were issued to 1

install the new wiring harnesses upon-receipt from the vendor.

The replacement work for the last diesel was completed on 12/24/85.

Findinos:

The installation method of amphenol connectors on actuators by the D/G manuf acturer was inadequate; however, TVA.and the manuf acturer acknowledged the problem and took the'necessary corrective actions.

These corrective actions have been implemented and no evidence exists of any remaining inadequate installations.

Although the incorrect installation of connectors could have W

contributed to an unreliable operation of the D/Gs, the j

problem was identified during a surveillance test (plant i

standard procedure required per Technical Specification) and j

corrected in a timely manner.

Conclusion:==

Although the employee concern is valid, the problem was identified independently from the concern during a required surveillance test as required per plant Technical Specification. Also the problem was acknowledged, corrective actions taken very expeditiously, and notification was properly provided to the NRC.
