HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING JULY 17, 1987 Shoreham ]

In denying LILCO's request to operate Shoreham at 25% of rated capacity on June 11, 1987, the Commission stated that, "LILCO may refile its request under 10 CFR 50.57(c) with the Licensing Board when and if it believes that some useful purpose would be served thereby." 1 North Anna Power Station On July 15, 1987, the North Anna Power Station declared an Alert (6:54 a.m.)

following a manual trip of the reactor from 100 percent power due to a steam generator tube leak. The NRC entered Standby Mode (7:26 a.m.), the Operations Center and Incident Response Center at Region II were manned and the Region dispatched a team to the site. The NRC returned to Normal Mode when the residual heat removal system was placed in operation as part of the controlled sequence to place the unit in cold shutdown which significantly der.reased the potential for offsite consequences.

Humboldt Bay Power Plant Unit 2 A memorandum of agreement has been signed between the joint interveners and Pacific Gas and Electric Co. (PG&E) on the Humboldt Bay Unit 3  ;

decommissioning plans. A stipulation that all contentions by the interveners are withdrawn and/or a motion to terminate the hearing will be filed with the ASLB by the parties to the proceeding. If the Board agrees, the hearing will be terminated.

The agreement between PG&E and the interveners states that PG&E agrees to: 1) ship the spent fuel assemblies to DOE as soon as DOE has a repository or some other facility in operation capable of receiving the fuel; and 2) complete final decommissioning of Unit 3 soon after shipment of the fuel and after receipt of appropriate approval from the NRC. The agreement also states that the interveners agree to withdraw their contentions.

Differing Professional Opi.nions In April 1987, the Office of Personnel assumed responsibility for administering the Differing Professional Opinions (DP0) program. A Special Review Panel to review DP0 policies and procedures and to review appropriate past DP0 submissions for possible awards has been established. The review is expected to be completed by December 1987. ..

McGuire Unit 2 From July 7 through July 10, an Augmented Inspection Team (AIT) was at the McGuire site to investigate the jamming of Reactor Trip Breaker (RTB)"B". This RTB failed to open in response to the controi room operator's attempt to manually open it during the performance of a control rod drop test while the Unit was in hot standby with all control rods fully inserted. The shunt trip coil remained energized and, thus, l heated up and burned out. The licensee replaced the "B" breaker with one bypass breaker from Unit 1 and continued mode 3 operations at Unit 2.

This RTB is a Westinghouse Model DS-416. The primary cause appears to be a cracked weld between a " pole shaft" and " center lever pin."

Similar RTB failures are known to have occurred infrequently. The AIT attempted to simulate without success the "B" breaker jamming.

Furthermore, the AIT inspected the control room switch circuitry to resolve a possible problem regarding the RTB light indication because the control room operators stated that the light had an "open" indication when in effect the breaker was closed.

Further evaluation is necessary to define the root cause of the jamming and possible generic implications. Westinghouse and the licensee plan to conduct further investigations of the failed RTB and the NRC Vendor Inspection Branch will followup on their actions. ,

JULY 17, 1987 ENCLOSURE B

. I McGuire Unit 2 l The following update is provided for daily highlights issued July 6 and 10, 1987 regarding the reactor trip breaker failure during rod drop tests at McGuire Unit 2 on July 2, 1987.

The Augmented Inspection Team (AIT) conducted an exit at the site on 7/10/87.

The team independently evaluated the licensee's and Westinghouse's investiga-tion into the reactor trip circuit breaker problems. The Unit 2 breaker failed to open during control rod drive time response testing. The breaker also erroneously indicated open in the control room.

Abnormal wear and a broken weld associated with the main drive link were found in the Westinghouse early vintage DS-416 breaker. Attempts to duplicate the condition where the breaker was mechanically binding in the closed position, were minimally successful. The breaker's electrical controls and auxiliary contacts were verified to be properly wired and properly operating, except for the shunt trip coil which had sho'ted while trying to open the breaker.

It was agreed by all parties that further investigation and dismantling of the breaker should be conducted in research laboratory conditions. Preliminary l conclusions by the AIT are that the breaker mechanical binding was caused i by a combination of high cycle wear, manufacturing tolerances in this vintage of breaker and a broken weld. These factors combined to allow sufficient lateral movement of the main drive linkage to cause it to jam and not allow the breaker movable contacts to open. The erroneous open indications was not attributed solely to breaker malfunction and field wiring is being verified.

The licensee has agreed to short term corrective actions until the long term investigation results are finalized.

Issuance of Order Modifying License to Convert from High-To Low-Encriched Uranium Fuel, Rensselaer Polytechnic Institute The first order to convert from high-to low-enriched uranium fuel at a i non-power reactor was issued on July 7, 1987 to the Rensselaer Polytechnic l Institute. The Order will become effective within 30 days of its date of publication in the FEDERAL REGISTER (52 FR 26191) on July 13,1987 or, if a hearing is held, on the date specified in an order following further proceed-ings on this Order. The Order is in response to 10 CFR 50.64, " Limitations on the Use of Highly Enriched Uranium (HEU) in Domestic Non-Power Reactors," which  !

became effective on March 27, 1986. The intent of the regulation is to reduce i' the risk of theft or diversion of highly enriched uranium fuel used in non-power

reactors and the consequences to public health, safety and the environment from i such theft or diversion. A total of 23 licensed non-power reactors are affected by this regulation.

.NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR JULY 13 - 19, 1987

1. DEFUELING l~ - Pick and place defueling of partial length fuel assemblies and fuel pins continued. Partial fuel assemblies E-3, F-2, F-3, F-4, G-3,.

G-4, and G-5 were loaded into defueling canisters. A total of 52 1 I

fuel assemblies out of 177 in the core have now been broken free i from the lower grid and loaded into canisters.. [

- Four canisters were filled and one partially filled. .None of the j filled canisters were transferred to the spent fuel pool.  ;

2. ENVIRONMENTAL MONITORING i

- US Environmental Protection Agency (EPA) sample analysis results ,

show that TMI site liquid effluents are in accordance with '

1 regulatory limits, NRC requirements, and the City of Lancaster Agreement. 1 TMI intake water and water effluents from the station (Units 1 and l 2)dischargeareanalyzedbyEPA. Gamma spectrum analyses of the. l seven daily composited samples for June 27 - July 4,1987 indicated i no reactor related activity.

- EPA's gamma spectrum analysis of the NRC's TMI outdoor air sample for July 9 - 16, 1987 showed no reactor related radioactivity.

- The water works for the City of Lancaster composited seven daily samples from June 28 _ July 4, 1987. EPA's gamma spectrum analysis of the composite sample showed no reactor related radioactivity. i i

3. DECONTAMINATION ACTIVITIES i

- Sludge removal frc:a the reactor building basement floor has been completed. Robots were used to collect the sludge and the thin sludge / water mixture was pumped to collection tanks in the auxiliary building. About 1,000 gallons of dewatered sludge will be solidified prior to being transported for disposal at a burial site.

An estimate of four 170 cubic feet liners will be required for the

! solidification.

Robotic scabbling'of floor areas in the seal injection valve room i (SIVR) on the 281 elevation of the auxiliary building has resumed.

l About one third of the total floor surface has been scabbled.

l. - Floor scabbling continues in the north and south valve alleys on the l 281' elevation of the fuel handling building.

5. ENVIRONMENTAL IMPACT STATEMENT  ;

The Programmatic Environmental impact Statement Final Supplement No. 2, NUREG-0683, dealing with disposal of accident-generated water was issued on June 30, 1987. In Supplement 2, the staff concluded that GPU Nuclear, Inc.'s proposal to dispose of the water by forced evaporation to the atmosphere, followed by onsite solidification of the remaining solids, and disposal in a licensed low-level waste disposal facility is an acceptable plan and will recommend to the Commission that it be approved.

The staff evaluated this proposal, together with eight alternatives, relative to the risk from radiation exposure to workers and to the general public, the probability and consequences of accidents, the commitment of resources (including costs) and regulatory constraints.

f The staff concluded that no alternative was clearly preferable to the GPU Nuclear, Inc. proposal.

An opportunity for a prior hearing on the staff's proposal to lift the

! prohibition on the disposal of the contaminated water is being' offered to

! GPU Nuclear Corporation and to other persons whose interest may be affected. Requirements for filing a petition for leave to intervene in any proceeding will be contained in a notice being prepared for publication in the Federal Register.

Riverton Remedial Action Plan '

l The staff concurred on DOE's proposed Remedial Action Plan for the Riverton Title I site on July 9, 1987. The staff found that the proposed relocation of Riverton tailings to an NRC-licensed mill, followed by restoration of the Riverton processing site, will comply with the applicable EPA standards. NRC  ;

concurrence is conditional with respect to two items: 1) 00E showing '

compliance with EPA groundwater standards when promulgated, and 2) DOE's contractor obtaining appropriate NRC approval for the disposal of the tailings l and adequate financial surety. j l

Low-Level Waste Data Base On July 8 and 9, Messrs. G. Roles and T. C. Johnson met with representatives of DOE, EG&G and States to discuss the information needed for entry into the  ;

DOE funded Low-Level Waste Data Base. This data base would be developed from disposal manifests and used for disposal site allocations, surcharge accounting and performance assessments. The States agreed that the manifest j information presented by DOE and NRC is needed. However, the States of Nevada, South Carolina and Washington could not commit to obtaining this information in an electronic format from the disposal site licensees.

Contracting directly with the site operators is an alternative which will be considered by DOE /EG&G. NRC staff plans to consult with 0GC staff and consider direct alternatives such as rulemaking to obtain the needed information.

Combustion Engineering, Windsor, Connecticut During the period of July 13-14, 1987, three representatives from NMSS:FCSB and Region I visited the Combustion Engineering (CE) site to become familiar with the facility and process. The visit was made part of the  ;

processing of an amendment application submitted in December 1986. As a 1 result of discussions before and during the visit, a new submittal will be j made. Other discussions were held concerning licensing actions for processing higher enrichments at the facility and license renewal issues.

The proposed rulemaking package was forwarded to the ED0 on February 9, 1987. In a change from prior policy, the E00 staff determined that this rule amendment must be brought before the CRGR. On May 27, 1987, the CRGR.

approved the proposed rulemaking for publication. The proposed rulemaking was published in the. Federal Register on June 26, 1987 (52 FR 24015). The comment period is scheduled to end August 25, 1987.

Storage of Spent Nuclear Fuel in NRC Approved Casks at Civilian Nuclear Power Reactor Sites (10 CFR 2; 10 CFR 51; 10 CFR 72)

The Nuclear Waste Policy Act of 1982 (NWPA) under section 218 (a) states, in part, that the Secretary [of DOE] shall establish a demonstration' program, in cooperation with the private sector, for dry storage of spent nuclear fuel at civilian nuclear power reactor sites, with the objective of establishing one or more technologies that the. Commission (NRC) may, by rule, approve for use at sites of civilian nuclear power reactors without, to the maximum extent practicable, the need for additional site-specific approvals by the Commission. Section 133 of the NWPA also includes the statement that the Commission shall, by rule, establish procedures for the licensing of any technology approved by the Commission under section 218 (a).

This new rulemaking is being prepared at.the request of the Director, NMSS, in a memo dated June 17, 1987. A draft rulemaking package is being prepared for submittal to the EDO for approval of initiation.

ROAB personnel described recently~ completed engineering evaluations of auxiliary feedwater pump trips caused by low' suction pressure.

TPAB described AE0D analysis techniques regarding 1986 reactor scrams and its operating experience feedback report--new plants, and NAS described its human factor study of wrong unit / wrong train events.

Incident Response Branch i

On July 14, 1987, following a loss of offsite power at the Palisades plant due l to an electrical fault in one of its three startup transformers, the Operations

! Center was manned for several hours by a small cadre of response team members-to monitor plant cooldown and restoration of offsite power. The unusual event and operations center manning were terminated late in the evening when offsite power was restored.

I On July 15, 1987, the North Anna Power Station declared an Alert (6:54 a.m.)

l following a manual trip of the reactor from 100 percent power due to a steam generator tube leak. The NRC entered Standby Mode (7:26 a.m.), the Operations Center and Incident Response Center at Region II-were manned and the Region dispatched a team to the site. The NRC returned to Normal Mode when the residual heat removal system was placed in operation as part of.the controlled sequence to place the unit in cold shutdown which significantly decreased the potential for offsite consequences.

2. A Further Action on Show Cause Order and Confirmatory Order Modifying License was issued to Kedarnath B. Joshi, M.D., Livonia, Michigan on July 10, 1987. This action is based on letters received from Dr. Joshi in January and May,1987, that Dr. Joshi will be taking an increased personal 3 interest in assuring that his activities are conducted in compliance with NRC requirements and audits by an independent consultant are covered in the Confirmatory order Modifying License.
