ENCLOSURE 1 N0ilCE OF VIOLATION Carolina Power and Light Company Docket Nos. 50-325 and 50-324-Brunswick 1 and'2 License Nos. OPR-71 and DPR-62 During the Nuclear Regulatory Commission (NRC) inspection conducted on January 1-31, 1989, a violation of NRC requirements was identified. -In accord-ance with.the " General Statement cf Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989), the violation is listed below:
Technical Specification 6.8.1.a requires the licensee establish, implement, and maintain written procedures.as recommended by Appendix A of Reg (ulatory Guide 1.33, November 1972.
Appendix Item A.3, Equipment Control e.g.,
locking and control), requires. procedures for clearances..AI-58, Equipment Clearance Procedure, Revision 22, implements this requirement..Section 4.1.3 of AI-58 requires placement of clearance tags as required and in the order specified by the clearance sheet.
In addition, it also requires that a second operator confirm that these actions have been performed.
Contrary to the above, an operator failed to place a clearance tag as required, in that on December 21,1987 -the 1A RHR pump breaker was racked-out instead of the. 2A RHR pump breaker, as required by clearance 2-1489.
Additionally, a second operator, who performed 'the clearance-confirmation check, failed to detect the error.
This is a Severity Level IV violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Carolina Power and Light Company.is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, DC 2055, with a copy of the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, within 30 days of the date of the letter transmitting this Notice.
This reply should be clearly marked as a " Reply to a Notice of Violation" and should include [for each violation]:
(1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the.results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
