ENCLOSURE-q pilgrim Status Report for the Period February 11 - 24, 1989 1.0 Plant Status As of 8:00 a.m. on. February 24, 1989, the reactor was.in the cold shutdown mode with moderator' temperature about'180 degrees Fahrenheit.

2.0 Power Ascension Test P'rogram Status The licensee completed the Power Ascension Test Program up to the 5% power-plateau.

At the completion of this testing, the licensee shutdown the reactor to perform maintenance and to.. await NRC approval to proceed with the Power Ascension Program.

Prior to continuation of the power ascension program to the 25% power E

plateau, the licensee must obtain the NRC Region I Regional Administra-i tor's agreement. The. program includes NRC holdpoints at 5%, 25%, 50% and-75% of full power and both a licensee written report and NRC review after completion of testing.at full power.

The NRC Restart Assessment ' Panel recommends to thel Regional Administrator when the plant is. ready-tof be released from each holdpoint.

Contingent upon NRC release.for operation up to 25% power,.the licensee will continue implementation of their approved test program. The following testing activities ' are scheduled for this next phase:

turbine generator overspeed tests, synchronize the turbine generator to the grid, ~ reactor core isolation cooling (RCIC) and high pressure coolant injection (HPCI) flow tests at 25% power and reactor mode switch testing.

3.0 Facility Operations Summary i

I-As reported in the last Pilgrim. Status Report (January 29 February 10,1989),

plant startup commenced on - February 10, 1989, following completion of modifications to the air supply for the vacuum breaker block valves. Main steam isolation valve (MSIV) testing performed at a reactor pressure of 270 pounds per square inch gage (psig) identified a need for some adjustments and repairs. The MSIV's function to isolate the main steam lines and limit the release of reactor coolant or radioactive materials during certain postulated accidents.

Repairs were b

made and testing was successfully completed.

Reactor pressure was subsequently raised to about 350 psig and the safety relief valves (SRV's) were tested.

SRV's provide overpressurization protection of the reactor vessel.

Pressure was raised to 600 psig for RHR system and drywell leakage inspections, and then to about 950 psig for HPCI and RCIC testing and additional RHR system and drywell leakage inspections.

The licensee successfully completed their planned testing activities up to the 5% power plateau and formally requested NRC release to proceed with power ascension from 5% to 25% power, on February 16, 1989.

On February 18, 1989, the licensee initiated a controlled plant shutdown to perform maintenance while awaiting NRC agreement to proceed.

Cold shut-down was achieved on February 19, 1989.

On February 21, 1989, at 1:00 a.m.,

a partial loss of offsite power occurred due to an electrical fault in a cable associated with the startup transformer.

This transformer provides normal station electrical power when the main generator is not in service.

The two station emergency diesel generators automatically started as designed to provide station

-power.

The licensee restored offsite power to the plant via a second transformer at 4:20 p.m.

The plant remained in cold shutdown for the remainder of this report period. Detailed NRC review of the above licen-see actions will be documented in Inspection Report No. 50-293/89-01.

Throughout the period, the licensee continued to conduct routine surveil-lances along with preventive and corrective maintenance.

4.0 Items of Special Interest NRC Restart Assessment Panel Meeting An NRC Restart Assessment Panel meeting was held on February 17, 1989, at Pilgrim station.

NRC management from Region I and headquarters were onsite for the meeting and Region I staff participated via teleconference.

The assessment panel received a presentation from the licensee on their assessment of the results of the 0-5% power ascension program. The restart panel also assessed the licensee's power ascension test program up to 5%

power and reviewed the licensee's readiness to proceed up to 25% power.

The NRC Restart Assessment Panel meets periodically to coordinate planning and execution of NRC inspection and licensing activities related to Pilgrim. The Panel is chaired by Mr. Samuel J. Collins, Deputy Director, Division of Reactor Projects, Region I.

5.0 Emergency Notification System (ENS) Reports Partial Isolation of the Reactor Water Cleanu.p_ System On February 10, 1989, the licensee experienced a partial isolation of the reactor water cleanup (RWCU) system.

The RWCU system removes dissolved impurities and corrosion products from the reactor coolant. The isolation occurred during plant startup and was caused by a false indica. tion of high system flow due to air in the instrument lines.

. Main Steam Isolation Valve Isolation On i February 11,1989, ~ the licensee experienced an isolation of..the main steam isolation ~ valves (MSIV's). The, isolation occurred during MSIV test-4 ing when the "C' outboard isolation valve was opened and reactor vessel level swelled to' about 44 ; inches, causing isolation ' of the MSIV's on a

.high reactor vessel water level trip signal. The isolation was reset and'

testing' continued without'further isolations.

Malfunction 'of the Drywell Airlock Door Interlock i

On February :16, -1989, an interlock designed - to prevent simultaneous open-ing of the two in.. series drywell air lock doors malfunctioned, ' allowing workers who were. entering the drywell to open both' doors at the same time for.about five seconds. The licensee has taken corrective action. to. pre--

. vent reoccurrence and-to repair the malfunctioning door. interlock.

Isolation of Reactor Water Cleanup System On. February 16,1989, - the licensee experienced. a RWCU system isolation from a ' sensed system high flow when.a -flow control. valve-was adjusted.

The probable cause was determined to be a brief system pressure fluctua-tion. RWCU was subsequently restored.

- Partial Loss of Offsite Power

.On February _21,'1989, the licensee experienced a pa'rtial loss of offsite:

power due to. an electrical fault in a cable associated with the startup.

transformer.

This' event -is described in detail in section ' 3.0 above.

Detailed NRC review of the events d9 scribed above will be documented in-L Inspection Report No. 50-293/89-01.

6.0 Pilgr,im Restart Staff' Activities The Pilgrim restart str.ff maintained around the clock shift coverage until

. February 17,1989 at 10:20 p.m.

This coverage was reduced to extended shift coverage consistent with the completion of the 0-5% phase of the power ascension te. ting program and plant shutdown.

Extended shift coverage was in effect at the close of this report period.

The shifts were staffed by personnel from NRC Region I, NRC Headquarters, and an NRC contractor. A specialist inspection in the area of transportation / solid radwaste was also performed.

The Pilgrim Restart Staff continued to evaluate the performan' e of licen-1 c

see personnel and the plant utilizing this inspection plan 'to determine whether the licensee should be permitted to proceed to the next NRC hold' point, a maximum of 25% rated power.

Contact with -the press and public continued throughout the-period. The

. NRC Restart Manager has been handling inquiries from the press and the public with the assistance of..the Region.I Public Affairs Officer.
