1.0 PURPOSE This instruction delineates how SAFETEAM investigations are I
conducted.
2.0 SCOPE This instrucElon applies to the activities associated with coordinating, investigating, and reporting investigations of Class 1 (Nuclear Safety or Quality) concerns received by SAFE"lEAM.

Appendix IV D, Documentation and Forms 5.0 RESPONSIBILITIES 5.1 The Manager, SAFETEAM Program has the responsibility for the implementation, maintenance, and updating of this instruction and keeping the SAFETEAM Review Committee advised of any significant changes.
The Manager, SAFETEAM Program is responsible for ensuring that a complete and thorough investigation is conducted of all Class 1 concerns received by SAFETEAM.

The SAFETEAM Investigator Coordinator will keep the Manager, SAFETEAM Program advised of any significant or noteworthy investigation findings immediately.

designee is responsible for assigning all Class 1 concerns, the initial review of completed draft investigation reports and informing the SAFETEAM secretary of the priority for typing investigation.

5.3 The SAFETEAM Investigator is responsible for conducting a complete and thorough investigation of each concern assigned to him/her and providina a thorough concise and legible Investigation Rcport to the Lead or Investigator coordinator, of the

Additionally, the SAFETEAM Investigator is. responsible for the preparation of an Investigation.. Plan for.each investigation conducted and completing a SAFETEAM Investigator Daily Report form, Appendix IV D, Daily.
5.4 The SAFETEAM Secretary is responsible for typing the draft and final investigation reports.
Final investigation reports will be prioritized by the SAFETEAM Investigator Coordinator for typing by the SAFETEAM Secretary.
5.5 The SAFETEAM Administrative Technician is responsible for computer inputting and tracking of all concerns and their associated or related documentation, e.g.
SAFETEAM safety concern record (s), investigation reports, response letters, recommendation letters, recommendation log, letters of transmittal and investigation concern file folders. Additionally, the Administrative Technician will attend all SAFETEAM Review Committee meetings.
The Administrative Technician will act as a backup for the SAFETEAM Secretary when the SAFETEAM Secretary is absent.

I SAFETEAM Program Instruction ST-03 SHEET OF SUREM DATE IMUED Class 1 Concern Investigation Process 7/03/87 6.0 REQUIREMENTS 6.1 After a concern ha's been classified, Class 1 in accordance with Instruction ST-02, the Investigator Coordinator will assign the concern to a SAFETEAM Investigator.

6.2 The investigator, within two (2) working days from receipt of a Class 1 concern, reviews all information provided with the concern.
6.3 If the Investigator determines sufficient information l
has not been provided to conduct a thorough investigation:
6.3.1 The Investigator will request in writing to the Interviewer Coordinator to gain additional information by contacting the concerned or I
scheduling another interview to get the additional information.
i 6.3.2 If this is not possible, a letter containing a request for the necessary information, is j
forwarded to the concernee by the Interviewer j
Coordinator.
i 6.4 An Investigation Plan, Appendix IV D, is completed by the Investigator and submitted to the Lead Investigator or Investigator Coordinator prior to j
conducting an investigation.
The Investigation Plan will contain as a minimum:

6.4.3 A list of documents the Investigator will use to conduct the investigation or to better i

6.5 The Investigator will submit the Investigation Plan to the Lead Investigator or Investigator coordinator or designee for discussion and approval.
6.6 An investigation into the concern (s) is conducted.
During the investigation, the investigator will:-
6.6.1 Request the assistance of the appropriate personnel as appropriate for technical support.
6.6.2 Contact the appropriate Manager (s) or Supervisory personnel, informing them that SAFETEAM is conducting a'n investigation in their area, and advise them that it may be

necessary to interview members of their staff, or craft personnel.
6.6.3 Make notes of those interviews conducted with all persons interviewed, utilizing SAFETEAM note pad.
6.6.4 Record the results of any processes or activity pertinent to the investigation.
6.6.5 Acquire copies of documentation for reference and file that will aid or support the findings of the investigation.
6.6.6 Resolve the who, what, when, where, why and how of each concern investigated.
6.6.7 Complete and turn in daily, SAFETEAM Investigator Daily Report to the Lead Investigator or Investigator Coordinator.
6.7 During the conduct of an investigation, there may be an occasion when a individual may give the investigator a HLP 2.43(08/85)

In the event of this, the investigator shold follow steps 6.'7.1 through 6.7.5 below.
6.7.1 First, encourage the individual to express his/her concern to the SAFETEAM Appreciation Center in person or by telephone,
- second, provide the individual with a SAFETEAM Concern Report package and ensure that he/she understands

the purpose and use. - Third,- encourage the individual to complete the package on the spot so that it can be delivered to the SAFETEAM Appreciation Center by you.
6.7.2.
If the individual fails to complete the SAFETEAM Concern Report packaage on the spot and give it to the investigator to deliver to the SAFETEAM Appreciation Cenfer.
The investigator should note the date, time, badge number (safety hard hat) and subject of concern.
6.7.3 The investigator will then provide the SAFETEAM Interviewer Coordinator with a written inter-office memorandum describing briefly the circumstances of the event along with the date, time, badge number (safety hard hat) and subject j
of concern.
l 6.7.4 The SAFETEAM Interviewer Coordinator will monitor l
walk-in, mail-in and phone-in concerns received during the next five working days.
If no concern is received that is the same or similar to the subject in the inter-office memorandum from the investigator, the coordinator will notify the investigator with a written memorandum that the concern was not received.
6.7.5 Upon receipt of the memorandum from the coordinator, the investigator will attempt to re-locate the individual and repeat step 6.7.1 above.

6.7.6 The Manager, SAFETEAM Program will review the documentation and specifically the subject of the concern and make a determination if SAFETEAM has enough information to investigate'the concern.
6.7.7 If enough information is available to conduct an investigation, the Manager will place the information into the SAFETEAM process.
6.7.8 If the information is not sufficient to conduct
,an investigation, the Manager will document the review and justification.
This information along with all other documentation will be forwarded to the SAFETEAM Administrative Technician for

filing.
6.8 When the investigator is confident that he/she has conducted a thorough investigation, and answered all questions raised in the concern, a draft Investigation Report of the investigation is written.
6.8.1 The investigation report will be formatted in accordance with Attachment A-1.
6.9 The draft Investigation Report is typed by the SAFETEAM Secretary and returned to the Investigator.
6.9.1 The Investigator reviews the draft Investigation Report for corrections, grammar, spelling and typographical errors.
6.9.2 The Investigator attaches to the report (corrected as necessary) all documentation associated with the concern (s).

Manager, SAFETEAM Program, Deputy Program Manager, and the Investigator coordir.ator copies of all approved investigation reports on the Wednesday grior to the Monday SAFETEAM Review Committee Meeting.

6.13 The Manager, SAFETEAM Program, Deputy Program Manager 4
and the Investigator Coordinator will review investigation reports for final approval, prior to transmitting the report (s) to the SAFETEAM Review Committee.
6.14 During this review, the SAFETEAM investigation report cover sheet will be annotated to reflect, if the concern was considered significant or not significant and if the concern was considered to indicate any wrong doings.
6.15 The SAFETEAM Administrative Technician will ensure that the SAFETEAM investigation report cover sheet is annotated properly after each meeting.
7.O DOCUMENTATION 7.1 Required documentation for Class 1 concerns may be in computer stored form, microfilm and hard copy file form.
