Inspection No. 50-353/87-11
This letter refers to the special safety Construction Team (CTI) inspection
led by E. H. Gray of this of fice on June 22 to July 2,1987, at Limerick
Unit 2.
Mr. Gray discussed the findings of the inspection with you at the
conclusion of the inspection.
Areas examined during this inspection are described in the NRC Region I
Inspection Report which is enclosed with this letter. Within these areas, the
inspection consisted of selective examinations of procedures and representative
records, interviews with personnel, measurements and observations by the
inspectors.
Based on the results of this inspection, it appears that certain of your
activities were not conducted in full compliance with NRC requirements, as set
forth in the Notice of Violation, enclosed herewith as Appendix A.
These viola-
tions have been categorized by severity level in accordance with the " General
Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2,
Appendix C (Enforcement Policy).
You are required to respond to this letter
and in preparing your response, you should follow the instructions in Appendix A.
In addition to the above, an apparent violation of NRC reporting requirements
under 10 CFR 50.55(e) was identified during the inspection.
The problem that
was not reported is the interaction between a non-safety fire protection detec-
tion system and the emergency diesel generators.
The system as designed for
Limerick Unit I would cause shutdown of the diesel generators should power be
lost and restored to the fire detection system under certain conditions.
This
situation is discussed in Section 5.4(e) of the enclosed report.
With regard
to this issue, we are considering appropriate enforcement action and have
scheduled an enforcement conference on October 13, 1987 in the Region I
office. At that conference you should be prepared to discuss the reason the
subject problem was not reported, as well as your corrective actions to prevent
recurrence.
