l APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No. 50-245 Millstone Unit 1 License No. DPR-21 During routine safety inspection from August 11 to September 8, 1987, the I following violation was identified in accordance with the " General Statement of l Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C.  !

Plant Technical Specification 6.8.1.c. reauires that written procedures l for surveillance of safety-related equipment be implemented.

Contrary to the above, surveillance of safety-related equipment under SP 4138, " Auto Blowdown Logic Test," was not implemented as required on August 12, 1987, in that Instrumentation and Controls (I&C) technicians failed to perform independent verification of correct test equipment connection as required by Steps 7.2 and 7.3. Also, surveillance of safety-related equipment under SP 408E, " Main Steam Line Isolation Valve Closure Functional Test," was not implemented as required , on August 13, 1987, in that I&C technicians failed to verify required initial conditions in accordance with Steps 5.2 and 5.4.

This is a Severity Level IV violation (Supplement 1).

Pursuant to 10 CFR 2.201, Northeast Nuclear Energy Company is hereby required to submit to this office, within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply including:

(1) the corrective steps taken and the results achieved; (2) corrective steps to be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause it shown, consideration will be given to extending this response time.

Under the Paperwork Reduction Act of 1980, PL 96-511, the response directed above is not subject to clearance by the Office of Management and Budget.
