Subject: Routine Resident Inspection 50-245/87-21 (8/11/87 to 9/8/87)
This transmits the report of the above subject inspection of Millstone Unit 1
by Mr. G. Grant of this office. The inspection findings have been discussed
with Mr. S. Scace of your staff.

Your . commitment to quality . maintenance and plant. cleanliness was reflected in-
the- findings of an August 18 inspection by the Regional' Administrator. The
.
good state of general housekeeping and radiological cleanliness .during the
plant outage were particularly noteworthy.
In general, our inspection concluded that effective planning and management
were demonstrated throughout the 1987 refueling outage. . Flexibility without
compromisMg quality or safety, as well as pride and excellence, were apparent -
at all levels of the outage team.

It. appears, however, that the activities set forth in the Notice of Violation
enclosed as Appendix A did not fully comply with NRC requirements.
You-are
required to respond.to that Notice of Violation and, in so doing, should follow
the instructions in Appendix A.
We have also noted that Millstone 1 inspection report 87-05, Detail 8 noted
handwritten clarifications and corrections in Rod Block Monitor Calibration
Procedure SP410C, with a quarterly check performed instead of the monthly
check. That problem, the two instances cited in the enclosed violation, and

the reactor scram caused by technician error on August 26, 1987 (Detail 5.b of
the enclosed report), cause us to question the recent adequacy of attention to
detail to Instrumentation and Controls (I&C) surveillance. We recognize that
excellent performance has also been identified by our inspections and that the
noted problems may be individual anomalies.
Nonetheless, bectuse such events
may also indicate a trend toward surveillance performance inadequacy, please
specifically address this concern with your reply to the enclosed violation.
