Subject: Resident Inspection 50-213/87-21 (July 15 - August 25, 1987)

This transmits the findings of the above subject inspection of the Haddam Neck  !

I Plant. These findings have been discussed with Mr. D. Miller of your staff.

The inspection identified a violation for failing to report a loss of spent

fuel pool cooling while the full core was off-loaded to the spent fuel pool.

Inasmuch as appropriate corrective actions have been taken in this case, no t

reply to this violation is necessary.

The enclosed report (Detail 6.3) addresses control room annunciator inoper-

ability for non-safety-related balance of plant equipment. Site management 4

,

took vigorous and appropriate action after our inspector questioned the timing

l of operator recognition of this condition. Our subsequent review concluded

that this was an isolated and non-representative instance of operator inatten-

tion. We do, nonetheless, wish to re-emphasize to you the importance of

vigilant operator attention to plant conditions.

The enclosed report (Details 6.8.2 and 6.9) also addresses unauthorized

activities by contractor personnel. We reviewed your immediate corrective

actions and found them adequate. Although station personnel made timely

discovery of these events, we are concerned that contractor personnel did not

comply with administrative requirements and failed to follow work orders.

Please reply to this matter, within 30 days of receipt of this letter, identi-

fying your corrective actions to prevent recurrence during future outages.
