Sumary.of Operations ,

The plant was first critical, following NRC authorization, on December 22. 1986, .

and was at about 15% power on December 23, 1986.  !

On December 23, 1986, while perfoming a special test of the ICS. to determine if the startup feedwater valve would properly control steam generator level, the feedwater valve closed fully, resulting in a low level SFRCS trip. The plant was at about 15% power at the time the trip occurred.

The unit was returned to power on December 24, 1986 and was at 40%.and synchronized to the grid on December 25, 1986.

On December 30, 1986, two unused taps for special instrumentation which had been capped in 1978, separated from the steam lines downstream of the turbine con, trol valves. The unit had been at 40% when a steam leak at one of the taps had been discovered. While reducing power in preparation for repair, the tap blew off. About an hour later, the second tap blew off. ' The cause of .

failure of the taps was detemined to be vibration induced failure of the weld securing the taps to the line. The turbine was shutdown and the lines were l repaired by plugging and welding.

Two similar taps which had not failed were removed and the line plugged and welded.

The turbine was placed back on line on Deceir.ber 31, 1986.

On January 1,1987, while the unit was at 40% power, with one main feed pump .

secured for maintenance, the remaining feed pump tripped as a result of high vibration. The cause was insufficient lubrication to a turbine bearing because.of a partial blockage of a lube oil line by a foreign ob. ject (a drain plug).

The reactor was restarted on January 2,1987 and is operating at about 52%

power. Repairs to the damaged feed pump will require at least 10-15 days.

PLANT ACTIVITIES All Mode 3 testing was compieced and the plant was restarted. Special restart testing was being undertaken in Mode 1, and will continue up to 100% power.

LICENSING ACTIVITIES NRR issued' authorization for the plant to restart on December 19, 1986.

PLANT NAME: Davis-Besse 1 PROJECT MANAGERS: A. De Agazio/L. Kelly Monthly Operational Overview for December 1986 NRR issued authorization for the restart of Davis-Besse on December 19, 1986.

The reactor was taken critical at about 0200 on December 22, 1986 and by 0930 the unit was at about 5% power. The plant was synchronized with the power grid oa Decamber 25, 1986 at about 15% power and by year's end was at approximately 50% power.

While doing a special test to determine if the new startup feedwater control system would properly control water level in the steam generators, the unit .

tripped from about 15% power,because the startup feedwater control valve shut.

The procedure beina used in the test subsequently was found to be in error,
