Gentlemen:

This refers to the inspection conducted by Messrs. P. H. Harrell, L. D. Gilbert,

and R. P. Mullikin of this effice during the period May 1-31, 1987, of

activities authorized by NRC Operating License DPR-40 for the Fort Calhoun

Station, and to the discussion of our findings with Mr. W. G. Gates and other 4

members of your staff at the conclusion of the inspection.

Areas examined during the inspection included operational safety verification,

maintenance, surveillance, plant tours, safety-related system walkdown, security

i observations, radiological protection observations, in-office review of periodic

!

and special reports, followup on previously identified items, followup on g

allegations related te welding, review of the program for installation of j

heat-shrinkable tubing, review of licensee actions related to reactor vessel  !

transient protection, verification of containment integrity, review of '

modification testing for installation of a containment equipment storage

platform, review of the licensed operator training program, and containment

local leak rate testing. Within these areas, the inspection consisted of

selective examination of procedures and representative records, interviews with

personnel, and observations by the NRC inspectors. The inspection findings are

documented in the enclosed inspection report.

During this inspection, it was found that certain of your activities were in

violation of NRC requirements. Consequently, you are required to respond to

this violation, in writing, in accordance with the provisions of Section ~2.201

of the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations.

Your response should be based on the specifics contained in the Notice of

l Violation enclosed with this letter,

We have also examined actions you have taken with regard to previously j

identified inspection findings. The status of these items is identified in i

paragraph 2 of the enclosed report. During review of these items, it was noted '

that three previously identified unresolved items related to the licensed

operator training program could not be closed due to inadequate implementation

of corrective actions. For this reason, it is requested that you provide a

response, within 30 days of the date of this '.etter, to the three unresolved

items discussed in paragraph 16 of the enclosed inspection report. Your '

response should include actions you intend to take to satisfactorily resolve the

items, and the date when your ac ions will be completed.
