Box 97, A200 Perry, OH 44081 SUBJECT: NRC OPERATIONAL ASSESSMENT TEAM INSPECTION REPORT 50-440/98007(DRS) AND NOTICE OF VIOLATION Dear Mr. Myers: On March 5,1998, the NRC completed an operational assessment team inspection at your Perry Nuclear Plant. The enclosed report presents the results of that inspection. . During the 2-week period covered by this inspection, your conduct of activities at the Perry facility was generally characterized by safety-conscious operations, and sound maintenance - and testing practices. We are concemed, however, about inconsistent control panel attentiveness by operators and reluctance by your operations staff and management to accept certain inspector observations. In addition, two violations of NRC requirements were identified. In the first instance, your staff identified a licensed operator incorrectly removed a red tag and installed a fuse for the wrong component. In the second instance, NRC inspectors identifed a more conservative design basis emergency diesel generator fuel oil tank volume calculation was not translated to the Technical Specifications document. .The two violations are cited in the enclosed Notice of Violation, and the circumstances . surrounding the violations are described in detail in the enclosed report. Please note that you -{ . are required to respond to this letter and should follow the instructions specified in the enclosed 1 Notice when preparing your response.
