NRC INSPECTION REPORT NO. 50-382/91-18

lhis refers to the inspection conducted by Messrs. W. F. Smith and S. D. Butler

of this of fice during the period May 14 through June 17, 1991.

The inspection

included a review of activities authorized for your Waterford Steam Electric

Station, Unit 3, facility. At the conclusion of the inspection, the findings

were discussed with those members of your staff identified in the enclosed

report.

Areas examined during the inspection are identified in the report.

The areas

included plant status, followup, onsite response to events, monthly maintenance

observation, bironthly surveillance observation, and operational safety

verification. Within these areas, the inspection consisted of selective

examination of procedures and representative records, interviews with personnel,

and observations of the activities in progress.

Within the scope of the inspection, no violations or deviations were

identified.

Three unresolved items are identified in the enclosed inspection report.

The first unresolved item (paragraph 3.2.1) will require further review by

Region IV radiation protection specialists to determine if the actions taken by

your staff were adequate in response to a potential radiation exposure in

excess of 10 CFR 20 limits. The second unresolved item (paragraph 4.1)

involves operators voluntarily entering Technical Specification 3.0.3 by

isolating the safety injection tanks to expedite depressurization of the

reactor coolant system during the reactor coolant system leakage event. We

have contacted the Office of Nuclear Reactor Regulation for assistance

ccncerning the proper use of Technical Specification 3.0.3 and 10 CFR 50.54(x)

during such eve ts. The third unresolved item (paragraph 5.1) will require

further review to determine whether there was a breakdown in the assurance of

quality of spare parts for the turbine driven emergency feedwater pump,
