SUBJECT:

NRC INSPECTION REPORT 50-341/98003(DRP), NOTICE OF VIOLATION, AND

NOTICE OF DEVIATION

Dear Mr. Gipson:

On March 16,1998, the NRC completed an inspection at your Fermi 2 reactor facility. The

enclosed report presents the results of that inspection.

During the nine-week period covered by this inspection, configuration control problems in your

345kV switchyard contributed to a series of spurious breaker trips, a main turbine trip, and an

i

automatic reactor scram Control room operators and plant equipment responded well to the

event. Investigations and corrective actions following the event were broad and comprehensive.

The subsequent forced outage was well-executed, including a number of emergent maintenance

activities such as the replacement of the B Residual Heat Removal Pump Motor.

Based on the results of this inspection, the NRC has detemnined that one violation of NRC

requirements occurred. This violation is cited in the enclosed Notice of Violation (Notice) and the

circumstances surrounding it are described in detailin the subject inspection report. The

violation was of concem because a thorough review of a safety-related procedure was not -

conducted before use, which resulted in the shedding of several loads.

Also based on the results of this inspection, the NRC has determined that two deviations from a

commitment occurred involving the failure to implement the specific regulatory positions

contained in Regulatory Guide 1.78, " Assumptions for Evaluating the Habitability of a Nuclear

Power Plant Control Room During a Postulated Chemical Release." The first commitment was

not met due to the failure to provide periodic training to operators to distinguish the smells of

hazardous chemicals. In addition, drills to ensure prompt donning of breathing apparatus were

not performed. The second commitment concemed respirators stored in the control room. The

number of respirators stored should exceed the number of operators in the control room.

However, only five respirators for five operators were contained in the control room.
