Gentlemen:

SUBJECT: NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/86-37 AND 50-328/86-37)

This refers to the Nuclear Regulatory Commission (NRC) inspection conducted by.

Mr. K. Jenison on June 6 - July 5,1986.

The inspection included a review of

activities authorized for your Sequoyah facility.

At the conclusion of the

inspection, the findings were discussed with those members of your staff

identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report. Within these

areas, the inspection consisted of selected examinations of procedures and

representative records, interviews with personnel, and observation of activities

in progress.

The inspection findings indicate that certain activities appeared to violate NRC

requirements.

Violation 50-327,328/86-37-03, references to pertinent

requirements, and elements to be included in your response are described in the

enclosed Notice of Violation.

An additional violation (50-327,328/86-37-06) was also identified and is

described in paragraph 13 of the enclosed inspection report.

This additional

violation is under consideration for escalated enforcement action.

Accordingly, a Notice of Violation addressing this particular violation is not

being issued at this time, and therefore no response to this violation is

required.

However, please be advised that the number and characterization of

violations described in paragraph 13 of the enclosed Inspection Report may

change as a result of further NRC review.

You will be advised by separate

correspondence of the results of our deliberations on this matter.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and its enclosures

will be placed in the NRC Public Document Room.
