INSPECTION NO. 50-334/86-12

Enclosed is the report of the team inspection conducted by Mr. R. C. Wilson and

other NRC representatives on June 9-13, 1986, at Beaver Valley Power Station,

Uni t No.1.

The team's findings were discussed with members of your staff at

the conclusion of the inspection. The inspection reviewed your implementation

of a program as required by 10 CFR 50.49 for establishing and maintaining the

qualification of electric ec;uipment within the scope of 10 CFR 50.49. Within

these areas, the inspection consisted of examinations of selected procedures and

records, interviews with personnel, and sbservations by the inspectors.

The inspection determined that you have implemented a program to meet the

requirements of 10 CFR 50.49. Six deficiencies in your program implementation,

summarized jn Appendix A, are classified as Potential Enforcement / Unresolved

Items and will be referred to the NRC Region I office for further action.

The most significant deficiency involves failure to demonstrate qualification

of Continental silicone rubber insulated instrument cable because no performance

test data were available for LOCA conditior.s. Other deficiencies involved

failure to show seismic acceptability for Rosemount transmitter installations

ard to fully documert qualification for the Victoreen High Range Radiation

Monitor, a Limitorque valve operator, a Barton transmitter, and certain types

of Rockbestos cable. One additional concern is classified as an Open Item,

and a future NRC inspection will review your actions concerning it.

Details

of all the deficiencies and concerns are discussed in the enclosed inspection

report.

Your letter of July 21, 1986 provides descriptions of actions you have taken

to address the Potential Enforcement / Unresolved Items. Preliminary review of

that letter indicates that, subject to verification in a followup inspection,

the Victoreen, Limitorque, Barton and Rockbestos concerns may be resolved and

you have made progress concerning the Continental and Rosemount deficiencies.

Completion of your corrective actions regarding the identified deficiencies

and concerns should not be delayed pending either a future NRC inspection or

further action by the NRC Region I office.

APPENDIX A

Potential Enforcement / Unresolved Items

As a result of the special equipment qualification inspection on June 9-13, 1986,

the following items have been referred to NRC Region I as Potential Enforcement /

Unresolved Items (paragraph references are to detailed portions of the inspection

report).

1.

Contrary to paragraphs (f) and (k) of 10 CFR 50.49 and Section 5.2.5 of

the 00R Guidelines, at the time of the inspection Duquesne Light Company's

(DLC's) files did not adequately document qualification of Continental

cable because performance during accident-testing was not measured and

shown to be acceptable.

2.

Contrary to Criterion III of Appendix B to 10 CFR Part 50, at the time

of the inspection DLC's installation of two Rosemount 1154 transmitters

was not seismically qualified because the installation differed from the

tested configuration and the effects of differences were not adequately

analyzed.

3.

Contrary to paragraph (j) of 10 CFR 50.49, at the time of the inspection,

DLC's installation of the Victoreen High Range Radiation Monitor was not

qualified because potentially high thermal-induced leakage current in

system cable precluded meeting the instrument performance requirements.

4.

Contrary to paragraphs (f) and (k) of 10 CFR 50.49 and Sections 5.2.2,

5.2.4, and 7.0 of the D0R Guidelines, at the time of the inspection, DLC

did not adequately demonstrate qualification of internal wiring in one

Limitorque operator because the wiring was not identified.

5.

Contrary to paragraphs (f) and (d)(3) of 10 CFR 50.49, at the time of the

inspection, DLC's installation of one Barton 764 transmitter was not

qualifted for its elevation below flood level.

6.

Contrary to paragraphs (f) and (k) of 10 CFR 50.49, at the time of the

inspection DLC did not adequately demonstrate qualification of Rockbestos

cable with compound KXL 780 insulation, which may have been used in the

plant.
