. April 30,1987. The inspection included 'a review of

activities authorized for your Browns Ferry-facility. At the conclusion of.the

"

inspection, .the findings were discussed. with . those members' of .your staff -

identified'in-the enclosed inspection report.

Areas examined during the inspection included operational safety, maintenance

obshnation,

surveillance

testing

cbservation,

reportable ' occurrences,

configuration management, Restart Review Board and the Independent Safety.

Engineering Group '(ISEG) activity, welding modifications, the lay up program,

and low' pressure turbine disc cracking,

Within these : areas, the inspection consisted. of selective examinations of

I

procedures and representative- records,

interviews with personnel,. and

observation of activities in progress.

The inspection findings indicate certain activities appeared to violate NRC

requirements.

The violation involved an : inoperable Control Room Emergency

Ventilation'(CREV) System.

The violation, references to pertinent require-

ments, and elements to be included in your response are described in the

!

enclosed Notice of Violation (NOV). For the first example of the enclosed NOV,

!

please ' address your reasons and corrective actions regarding the timeliness

a'spect of identifying . the actual low flow condition on the B train CREV

system.

Your attention .is. called to paragraph 7, Surveillance Testing observation,

where the Browns Ferry (BFN) quality assurance staff recently performed a

technical review of the BFN Surveillance Instructions (SI). The intent was to

i

.' review ten percent of the 430 sis but was temporarily halted after about five

percent because it was concluded that the existing technical adequacy review

program was inadequate. A condition adverse to quality report was written and

the SI program was halted.

More management involvement in the implementation

of this p_rogram will be required to assure satisfactory completion of the

. program.

Engineering Group (ISEG)' staffing level at BFN.

As described in the BFN

Nuclear Performance _. Plan,: Volume 3, dated August 28, _1986, ..the BFN ISEG would

consist of three full-time engineers onsite.

NUREG 0737, Item I.B.1.2 speci-

fies that the ISEG should be comprised of a minimum of five dedicated, full-

time engineers.

However, a number less than five may be acceptable, but must

be_ justified'on a case-by-case basis. Our concern.is that based on our current

knowledge _of :the qualifications you intend to use in filling. the BFN ISEG

.

positions, the experience level 'does not appear to justify going to fewer

than five members. Since the ISEG staffing issue was also. raised in our_ review

of the BFN' Nuclear Performance Plan, you should ensure that your revision to

)

Volume 3' addresses this question.
