It is 6 The next thing I would like to do is introduce 7 everybody. My name is Frank Hawkins, I am a reactor inspector 8 from the Region III Office in Chicago. To my left is Claude 9 Johnson who is a reactor inspector from the Ecgion IV Office to in Dallas and, of course, Mr. Cory Allen whom we are here to

12 Before we get started, let me go through the purpose 13 of the interview, and that is to clearly define cach 14 technical concern you have. I want to understand exactly

. 15 what your concern is in every area. We want to see everything 16 from your perspective.

17 Now, to accomplish that, we are g61ng to go through is each concern with you. To refresh your memory, we will read 19 it to you, in other words. Then we will discuss each one 20 with you.

7 4 O And I guess I am having a little trouble understandinq j 'under what circumstances the new tech coatings will be applied 4

in -the sequence that you are referring to, and that CCP-40 i refers to.

l II In other words, is it a repair or a standard i

A Well, it really doesn't state whether or not it is l 14 i

a repair or not. My assumption is it would probably be in a

-1 16 repair situation. But that is not clear in the procedures.

17 O Okay, so let me understand, then. Your concern is that the procedure of the specification that refers to the use of that coating does not specify or stipulate whether it is a repair or standard application.

A Right, that's my understanding.

O But to your knowledge, which is it typically used

j A I am not exactly sure. I think it is a repair 3 .

3 0 Okay. Now, let's assume that you are right and i

l 4 the intention is that it was to be used as a repair. The 5 procedures, I guess, control the technique used and 'the f 6 transitions, all that sort of thing, for repairs.

! 7 A Right.

i 8 0 How big an area would you be referring to where

9 that resulting system probably was used?

! 10 A 1 don't know. I don't think the procedure even 11 specifies or limits the actual area in which that system J

J 12 is used.

l 13 0 okay, but if it were repair, I guess that it would 14 b2 limited in size or extent because it was a repairs right?

- 15 Iri other words, an engine have that, according to spec --

i 16 or an engine happened, with, I should say. Do you recall l 17 that?

i 18 A 1 don't recall that. But let's take, for instance i

i 19 a repair area on steel. A repair area can be totally re-l 20 stripping entire piping. So, if it was a concrete substraight 21 they might strip, you know, a fairly large area all the way I 22 down to, probably 11-S and then come back and apply 1201,  !

1 23 11S/1201 or whatever.

l 24 I don't think there is any stipulation as far as

25 how large the area is.

t Q Okay. How could they strip of f the 1201 and leave  !

I 2 a clean 11S to work with? Wouldn't they use a roto pean or i 3 something and go right down to repair a substraight, and then j 4 start over from the beginning again?

$ A Sure, they can do that. But I am sure they i l l 6 probably can strip it down just to 11S. They keep stripping i 7 to remove all the white top coat until they are down to 11S.

i 10 0 Okay, and then they bring that back up to the <

{ 11 proper thickness and then apply these other systems?

l 12 A In that sequence, yes. In that sequence.

1 j 13 0 Okay. Have you ever seen that sequence used in the  :

I 15 A No, I haven't because I am not a certified concrete

17 0 okay, you are just a steel inspector.

1 18 A Exactly.  ;

4 19 0 Who told you that this system is actually being so used or has anybody told you that?

l 21 A Nobody has told me. I am just reading the I

22 Comancho Peak construction procedure, and there it is, i

M written in their procedures. And that is what my question j 24 is about, why is it in their procedure?

t I 26 0 Okay. So, your concern is based on your review of I

1 the procedures and the specifications.

1 2 A Exactly, right.

t 3 0 You say in your statement that the coating systems j

have not been qualified. What is the basis for that con-o clusion?

6 A Well, based on the manufacturer's letter. It says, l 7

"Although the resultant systems have not been qualification i e

tested, there is no reason to believe they are not viable That is the manufacturer that is actually saying g l systems."

11 0 To your knowledge, was the testing that was 12 l requested in the last paragraph in the May 1978 letter from 13 Imperial Coatings ever performed?

l 14 A I have no knowledge one way or the other.

16 Let's see, it was Attachment 2 to Exhibit 1. It is the last i

I paragraph here.

17 is It says, " Test will be initiated to demonstrate -

19 the intercoat adhesion proporties of these systems. It is

! 20 also suggested that El Cometer adhesion test be conducted by j

21 OC at the job site to confirm the expected good adhesion 22 between top coat and surface."

23 To your knowledge, was that ever done?

f 24 A No, I have no knowledge of whether it has boon done 25 or not. But again, the qualification testing I am talking

! 4 ANSI 101.2, Section 5, which is the section on repairability s tests.

i s was, would the testing that they specified in that last

, paragraph of the letter that we just read, would that I 30 testing have been suf ficient to qualify the system, in your I

) 12 A Well, that's a dif ficult question to answer because f 4

l 13 I don't know exactly what testing they are actually doing.

14 You have to evaluate what the test procedure was and how the i l

! . 15 test panels were prepared, et cetera, et cetera and you have [

to to know what the acceptance limits are.  !

17 So, I would have to review the actual test data *

1 is myself, plus the test procedure, in order to be satisfied in i i

19 my mind that it satisfied ANSI 101.2.

a) O So, let me make sure I understand what you have l I

4 21 told me. You want to know whether the systems, the two 22 systems we referred to earlier, have been qualified in i 23 accordance with ANSI 101.2 and 101.2.

l 26 0 That is the question. And when you referred to 101.3  !

2 A Exactly.

3 O The whole smear.

4 A Yes, sir.

5 0 All right. One thing I am not clear on, if this 6 were a repair system and it was limited in its use as far 7 as extent, would not the testing for intercoat adhesion be 8 enough to qualify the resulting repaired system, or the 9 resulting system as a result of the repair?

10 A Are you saying just El Cometer adhesion pulls to 11 determine?

12 Q Yos.

13 A I don't think so because you are still modifying 14 the system and overy time you modify the system you must go

. 15 back and perform the testing all over to make sure that this 16 modification doos not compromise the integrity of the 17 coatings. That is my understanding.

18 0 Okay. But by adding the extra two coats of 11 or 19 115 and 1201, wouldn't the likely result of that be a loss

2) of adhesion betwoon coats and not necessarily a result of 21 blistering or somothing like that, that would como up as 22 for as irradiation?

1 2 A Well, my foolings about that is that you are making i

24 educated guessos, you are making assumptions, and this all 25 has to be proved. As a coatings engincor, I would never want

1 j 1 to make a judgment of that kind. You would have to go back ti 2 and retest the system to make darned sure that it is actually i 4 i 3 going to work under the conditions of this project, under  :

i 7 0 You are concerned that the resulting repair

! t j 8 systems for concrete coatings,' which are specified in the I 9 procedure, were not ever -- to your knowledge -- qualfied in 1

l I 17 Are you assuming there had not been any test run, or is thi's 18 hearsay, or what?

! 19 A Well, it is this Imperial letter. They state  !

\ l 80 that they have not been qualify tested; that is their own 21 quotes. That is right out of their letter to Comanche Peak.

q 22 With my experience previously at the South Texas f I

23 Project, we used the identical Imperials coating system and 1l 24 we did not have -- I have questioned the Imperials lab for l

l 36 their test data and they never had any kind of repairability

21 Incorporated. I report to Tom Brant, who in the QC's 22 supervisor. I am a certified coatings inspector, certified 23 to inspect the application of coatings onto steel substrates.

34 My job is primarily inspection for hold points immediately to prior to application of coatings at Comanche Peak, and that

is for all steel surfaces.

4 A For liner plate, pipe hangers, conduit supports, 5 et cetera.

s Q Do you inspect just service level one areas?

l 7 A Exactly, only service level one.

8 Q Is QC at Comanche Peak involved at all in i

, inspection of service level two?

10 A No.  :

17 what you havo got there. "There seems to be a discrepancy' l f

te with the sequencing of coating systems at CPsts. Non- l to conformanco report C83-01752 questions the legitimacy of to applying carboline C2-11 over carboline 305. Note that f 21 this deposition for this NCR suggests that the coating  !

t 22 specification, AS-31, does not require a specific sequence [

23 of coatings for the system."  !

24 Could you explain the stateinent, Cory, "This to NCR suggests that the coating specification AS-31 does not l

15 2-3 1 require a specific sequence of coatings for the system"?

2 A Well, to explain that, if you look on Attachment 3 2, which is NCR C83-01752, reported by the 4 disposition, the second sentence says, and it is referring 5 to AS-31, the specification, the disposition says "This 6 table does not identify a full system sequencing or applica-7 tion parameters."

8 What we are talking about here is the coating 8 specification, the Gibson-Hill ccating specification that to is the parent or mother document of all the DC procedures, 11 the construction procedures. What the dispositon is saying 12 is that it does not identify full system sequencing or appli-13 cation parameters.

14 In other words, what he is telling us is that 15 they can apply any sequence of the ' coating systems they 16 desire, like primer with a tcp coat, the primer, the top 17 coat, whatever, that there apparently is no limit or restric-18 tion on what the sequence on the sequence of the coating 4

18 system they may apply at this job site.

20 0 To your knowledge, in this just for repair?

21 3 Well, this NCR deals with repair and there are 22 several other examplos that indicatos this is for rep. sir 23 only. l 24 0 I mean do you know of any other circumstancos 25 that this CZ-11 is applied over phonolino 305 besidos repair?

16 I A Well, it is not specified for the CZ-11 to be l

i 2 applied over 305, but it is done just whenever there is a  ;

3 violation of procedures by the crafts. But as far as 4

being specified, no, this is the only instance where it is 6

actually specified.

" So to your knowledge right now the only instance O

k is on the repair.

8 A Exactly. ,

O So what your concern is that the procedure is 10 not specifying whether it is for repair or not, am I correct?

1 A Well, I want to clear up that it is not really 12 in the procedure itself. It is only through this NCR we t

' 13 are talking about and the OC inspectors use this NCR and

! the craf ts use this NCR as well as the RFIC, which I would - [

- 15 like to introduce into the record. It is how the craft used l 16 j .

this to apply the CZ-11 over the 305 in the repair systems.

17 0 Okay, over what size area does the application 18

, occur, to your knowledge?  ;

19 j A Well, the actual interface is usually around

20 l two inches, maximum, two inches or so around the interface i

} 21 l of the repair area.

7 THE WITNESS: Yes. So it should have already 8

been provided.

9 MR. HAWKINS: We will go ahead and make it to Exhibit 2.

4 12 i O How should a repair interface be properly effected 13 as far as you are concerned?

A Well, how should it be -- I am sorry, I didn't understand your question.

O In other words, how would you perform a repair area?

18 A Well, the way I would probably do it is I would mask off the 305 and spray the primer directly over the steel so that the zink is not actually going over or

, overlapping onto the 305 which is allowed by that RFIC in l

this NCR.

, O I mean saying being realistic on this on the j 24 2

i small areas like that, to get a perfectly repairable surface 4

like that you would almost have to have say a perfect square 2

around that particular area to get prime coat and then top 3

coat applied. That is not practical construction practice, is it?

4 A Well, yes, it is because a lot of the repair 5

6 areas are being performed with disk grinders and whatever, 7

and they usually make a very circular or not straight surface, 8

but it is a circular or curved in appearance, and when you 9 are masking off with tape, it is my experience that you 10 definitely can cover that area exactly with tape.

11 0 Let me ask you this then. You are saying mask 12 off that particular area.

13 A Yes.

g4 Q Now how are you going to remove that particular 15 system? Are you going to sand blast it or are you going 16 to use some type of power tool?

17 A They are going to use a power tool cleaning f*r o 18 repairability.

gg Q Can you get a power tool to actually get a perfect.

20 square like that?

21 A What do you mean a perfect square? Just because 22 you are masking doesn't mean you have to have a perfect 23 square. It can be any shape or whatever.

24 O Then how are you going to prevent an overlap of 25 these materials?

I A That is what I am saying. You put tape over 2

the 305 so the CZ-11 isn't applied over onto the 305 top 3

coat. It is very simple to do with tape.

4 0 I guess what I am trying to ask here is to prepare 5

this particular area ---

6 A Yes.

7 0 ---

give me an example of what tool and what 8

method would be used and then go on through the masking 9

and taping. I would like to get a clear-cut idea.

10 A Say, for instance, you have a damaged area where

! the total system has been damaged like there is a cut or 12 something, you would probably get a disk grind or gripd.it 13 down and probably then you would get needls gun to roughen 14 up the surface. You would use the disk grinder to remove 15 all the coating and you would a needle gun to provide the 16 roughened surface. You would mask around the 305 so all .

17 you have is bare steel. You would spray the CZ-11 or brush 18 the CZ-11 onto that and then you would pull up the tape and 19 sand it down so you have a smooth transition and there you 20 are. That way you have never violated the coating system 21 as it is intended to be applied and you have t.he correct 22 sequence of coatings.

23 Q Are you satisfied with the provisions in Section 24 4.4.2.0 CCP30 for coatings and interfaces?

25 A Definitely not.

2-8 1 MR. HAWKINS: And you feel the method you just 2 specified is the way to do it versus this?

3 THE WITNESS: No. I am just responding to his 4 questions. I am not trying to say I am right or ---

5 MR. HAWKINS: No, don't misunderstand what I 6 asked you. I want to know another way to do it and I want 7 to get a feel for where you are coming from and want to 8 understand what your approach would be if you were doing 9 it or you were writing the procedure versus what is in there 10 now.

11 THE WITNESS: All I am trying to state here is 12 that the sequence, the actual coatings, I don't believe that 13 is correct. May I introduce another document?

14 MR. HAWKINS: Sure.

15 THE WITNESS: Here is an RFIC I sent to the 16 QE, Mike Foote.

17 MR. HAWKINS: This one is attached already to 18 Exhibit 1. So I know it is there.

19 THE WITNESS: Okay. If you will notice what 20 this is saying, this paragraph 44430 ---

21 MR. HAWKINS: Why don't~we identify it. It 22 is a Request for Information or Clarification dated October 23 20th, 1983.

24 THE WITNESS: And it is addressed to 25 who is the QE. The coding is QE.

21 2-9 1 MR. HAWKINE: From you?

2 THE WITNESS: Yes, from Cory Allen. I am 3 introducing this to show the cxmtradiction between the RFIC 4 we just spoke of, which is attached to an NCR and this RFIC 5 dated 10/20/83. What it is saying is that does this para-6 graph, paragraph 4430, does this paragraph allow CZ-ll or 7 Carboline 191 to be applied over existing phenoline 305 8 top coat and left intact without sanding back to a mottled 9 transition?

10 No. 2 of this RFIC says does this paragraph 11 allow phenoline 305 applied over reactic- 1201 or vice 12

versa ? The answer by is yes, this transition 13 interface shall be feathered back to assure a smooth final 6

14 coating system.

. 15 Well, this paragraph 4430 is extremely confusing 16 to the inspectors because we were using this to allow them 17 to apply carboline 191 and CZ-11 over 305 without them 18 sanding back. It is lef t intact and what you h'a ve is a 19 sandwich effect. You have CZ-11, 305, the CZ-11, 305 system 20 or you are allowed CZ-11, 305, carboline 191 and phenoline 21 305. You have about four or five different coating systems 22 right in a row that are being applied.

23 This paragraph is so confusing and so ambiguous 24 that we really don't know what to do, but what we are allowing 25 them to do is apply the sandwich effect.

22 2-10 1 MR. HAWKINS: Okay. Why don't we go ahead and 2

make that Exhibit 3 anyway, the October 20th, 1983 RFIC.

7 MR. HAWKINS: On here I want to make a little xxxxxxxxx sketch and I want you to explain something to me.

9 Here is the substrate, this is ideally, this 10 is the primer and here is the top coat. CZ-11 here and 11 phenoline 305 here.

12 Now draw for me exactly what happens during a 13 .

repair and how that is sanded back. Now isn't this cleaned 14 all the way in the center here, isn't this cleaned all the

- 15 way down to the steel strate?

16 THE WITNESS: Exactly, bare steel, except that 17 I think we should note that in the substrate itself profile 18 you are going to have existing CZ-ll.

19 MR. HAWKINS: That is the tight shadows and 20 tightly adhered residue that is referred to in SP-10.

21 THE WITNESS: Yes and we will get into that a 22 little bit later, but that is more or less addressed in 23 SP-10. What happens is you will feather this back to give 24 a tapered look right there.

25 MR. HAWKINS: And the width of that is what,

23 2-11 about an inch and a half according to procedure CCP-30?

1 THE WITNESS: Well, this distance, this interface 2

distance should be within an inch and a half, correct.

3 But what will happen, and let's take, for instance ,

4

. Carboline 191, they will apply the 191 over this distance.

5 There is the one inch and a half.

Then for the total system 6

7 you will come back and apply phenoline 305 over this.

MR. HAWKINS: Well, draw it in here where it 8

9 is really going to go.

11 THE WITNESS: That is the 191 which is the color 12 red, and then you will come back over the entire repair area 13 with 305. 4 MR. JOHNSON: I guess my problem is that it would 5 occur to me that QC would even have a bigger problem on this 6 of identifying which areas. You said inch and a half.is 7 difficult for OC to observe, but this method also seems 8 like it would be a little complicated also.

9 THE WITNESS: What, the method I am saying to 10 you?

11 MR. JOHNSON: Right.

12 THE WITNESS: No , not really because what I 13 am saying, and which they have been doing with the CZ-11 14 more or less, is that when you do this, just looking at it 15 from above, you have primer right hera and then you will 16 have a very small ring of exposed steel and then you will 17 have 305. That is after the repair has been performed. They 18 will have a repair area like that and bare steel with 305 19 going around it like that.

20 Then they will apply CZ-ll over this entire 21 area like this and then they start sanding back. They keep 22 sanding back until they find bare steel with CZ-ll in the 23 middle of it. Then when they have gone past the 305 and i,

1 24 hit bare steel, they know that the CZ-ll is not over the 25 305, they are on bare steel and they have gotten to the point i

2 MR. HAWKINS: All right. So I basically under-3 stand your concern to be, and I think we have about beat 4 that one dead, your concern basically is that the method 5 that they are using to effect repairs is not acceptable 6 because of the resulting systems that would_ leave a primer 7 over the top of a final finish coat in roughly an area of l 8 one and a half inches in width and varying circumferences.

9 THE WITNESS: Yes, but I would like to emphasize 10 that the repair area as stated in that paragraph deals 11 with an inch and a half. It seems very, very small, but 12 when you consider all the hundreds of repair areas that just 13 happen daily, it amounts to quite a lot of square footage 14 of coatings.

per hundred squ&re foot, and the ones with the average of 9

three? Could you do it that way and what the requirement 10 was basically to be within and .5 was out of?

11 THE WITNESS: It has been a while, but the 12 minimum thickness at that point in time, you were allowed 13 a low reading of 1.5 mils and a high reading or a maximum 14 reading of 5.5. What you do is you take five readings

- 15 per 100 square feet and you report the low reading, you 16 report the maxizum reading and you report the average 17

- reading. And 1.5 mils was probably the low reading, the 18 minimum or the low reading he was reporting.

words so I can make sure I understand what you really mean?

6 A It goes back to I believe it is the very first 7

concern we dealt with, this coating system of 11S/1201/11S/120 8

What it is is it is like four different coatings that make g up the total system and if you go by the allowable range 10 for each coating system and add them all up, you will come 11 out with over a hundred mils. That is part of the problem 12 of using this so-called stra6ge sequencing of the coatings, 13 is that when you start piling one coating on top of the 14 other, you come up with just an enormous film range, and

. 15 I just wanted to bring that out. That is another problem 16 with it.

17 0 Okay. Show me where you have come up with -

18 your over 100 mils. Maybe I am looking at this wrong.

19 We talking about 4.3.1.2, and if you come down here and a you add these mils up, the maximum, and now tell me where 21 we are getting the 100 mils in there.

22 A Say, for instance -- well, let's go back. Okay, a add the maximum for 11S, and that is 35 mils.

25 A Add the maximum for 1201, and that is 16. Add

I have a problem with that and I 23 would like to bring that to your attention to maybe investigate 24 that response even further because I am not convinced that 25 that is a sufficient answer to my NCR. Water will_ flow out

2 That is why I am writing this NCR because water is definitely 3

going to flow out of there and it is going to flow back --

4 when water flows in that building, it is going to first.

be investigated further.

- 15 A I honestly do not know what an acceptable response 16 would be, except to somehow -- you know, I have given this 17 a lot of thought and I don't know how to resolve this. If -

18 they cannot somehow mechanically trap the coatings, or at 19 least include those coatings in the total allowable amount m of coatings to have failed, in other words, the exempt coating s 21 law, they should probably restrict the coatings from all 22 items in the reactor core cavity.

23 Q To your knowledge, do you know that they haven't 24 performed some type of analysis as far as flow paths and 25 this type of thing and taken this into consideration?

any flow paths. I do not know that for a fact, but from --

3 well, my understanding is that they have contacted other 4

projects and from that regard it is more or less that they 5

have assumed that the coatings will not reach the containment 6 sumps.

A Oh, I know it hasn't been done.

you have done? Would you have done anything differently?

9 A Well, I lost one job over this. What I would' 10 probably do at Comanche Peak, I would probably exclude the 11 coatings, you know, just exempt the coatings and put them 12 on an exempt log. But in most cases it is my understanding 13 that it is essentially impossible because there is so much 14 square footage you would run over your allowed maximum 15 square footage of coatings. It would put you so far over 16 that you can't do it. So I think the answer would be to 17

. trap the coatings which I don't know how feasible that is, 18 and there is always the alternative of stripping the 19 coatings.

20 MR. JOHNSON: Do you have anything, Frank, 21 that you want to add?

22 MR. HAWKINS: No , I sure don't. We are just 23 looking at the resolution of that. I understand your 24 concern. It is a good question. )

12 My group is required to use "unsats" on 13 inspection reports to report deficiencies rather than NCR's.

14 I do not believe this is proper because NCR's are disposi-15 tioned by engineers, whereas IR's can be dispositioned by 16 anyone. An inspection deficiency reported on an IR 17 restricts what criteria the inspector is allowed to inspe'ch.

18 What coatings QC inspectors write "unsats" on IR's, Brown 19 and Root craft superintendents regularly complained to the 20 QC leads. I feel pressure from my superiors every time I 21 write an "unsat" on an IR.

22 BY MR. HAWKINS:

24 A I have written a couple in the last month or 25 so, but this was because I was instructed to do that.

an engineering review and all you did was identify it on 3

an IR?

4 A Yes, sir. Probably around September of this 5

year I started writing a notation on an inspection report 6

indicating that all traceability of the coating material 7

was lost- due to the uncontrolled handling by the paint 8

department. We had a situation where the paint would show g up inside the reactor building, but painters would then go p) and pour the paint into a bucket and walk off until they 11 were ready to pahlt. Therefore, you are losing all traceability 12 and there is no ID or anything on that paint and the inspecto r 13 was left to try to verify where the paint was coming from 14 or, you know, what mix and how it was and all that.

. 15 I brought this up in two different meetings with p5 my supervisors and lead men and told them about it and 17 explained it to them, and they ignored it. So I proceede'd' 18 to write this notation on every single inspection report gg for the last two or three months. We had a new supervisor 23 hired on and he just happened to see one of my inspection 21 reports and saw that notation and instructed me to immediately 22 write up an NCR against this.

23 So I have since written the NCR. I have 24 included about 12 different inspection reports noting "unsat" 25 inspection. He told me to go back and "unsat" the inspection l

3 The thing that I would like to bring out'is 4 that this not only deals with Cory Allen's inspection over 5 the last couple of months, but every single inspection 6 that has been performed in the last couple of months, i 7 Do you have anything besides paint traceability?

tool surface has been brushed or vacuumed. That has never 20 been performed on power tool cleaned surfaces.

21 So what a lot of the inspectors are doing, U

probably about 35 or so, are checking that " sat." I am checking it " sat", too, but I am writing a notation on 24 a

each inspection report that says surface was wiped with 25 a cloth rag, meaning that we are not performing it like ,

for quite a while is that the wiping immediately prior to 10 repairing surfaces like coming back and power tool cleaning 11 surfaces, they are wiping paint with foreign cleaning 12 solutions. They are using a hospital disinfectant over the 13 coating surf aces and these solutions carry a pretty high ,

34 percentage of chlorides in them, two percent chlorides. ,

1 15 The cleaning solutions are not specified in the j l

16 Procedure anywhere. The only thing they are allowed to use gi l'

17 is P henoline 305 thinner. So they are using a cleaning l

18 solution that is not specified.

4 About five or six months ago, I discovered - '

5 one of our tests we do is perform an air acceptability test.

6 We check the air as it comes out of a spray gun and make 7 sure there is no water or contamination in the air which 8 would prevent the actual curing of the coating.

9 I discovered one way or the other that the painters 10 were putting a cigarette butt in the cheater valve of their 11 spray guns. I found out this had been going on for quite a 12 long time at Ctrnanche Peak.

13 The reason they do this is because we do this 14 air acceptability test and there was so much water that was 15 going through the air lines that they would put this 16 cigarette butt immediately prior to our inspection -- behind 17 our back -- they would put the cigarette butt into the 18 cheater valve.

19 When we would go to check the air, it would be okay.

20 We would hold the gun up there against a pad of white paper 21 and check br 30 seconds, walk away, "It's okay." After we 22 would leave, they have to pull the butt back out because M there is so much water that it saturated that filter, the 24 spray gun would no longer work.

25 So, they pulled the cigarette filter out and would

2 O Yes, but the basic philosophy behind OC inspection 3

is that you can inspect those attributes at any time during 4 the process. Why didn't you go back in later on and do it 5 then? #

6 A You mean go back and check the gun to see if the 7

cigarette filter was in there; is that what you are saying?

8 Q Yes, if it's got one in there, identify it.

9 A Well, there is no question. I mean, I determined 10 it was in there. It was well known.

11 O And make them clean the lines out.

12 A Well, let me go further, the reason I didn't do 13 this. Upon finding this, I talked to the general foreman.

14 I questioned him about it. He said, "Yes, this is true."

- 15 I said, "Okay," and I went and picked up the telephone and 16 17 called my supervisor

I brought him out to Elevation 905 in the reactor 18 building with I explained the entire 19 situation to him. He said, "Cory, I see no problem with it.

3 It was a very, very important RFIC and for some reason it 4

. 7 A It is a very, very, very long time because what a happened was, I was trying to write it so that I could "unsat" g items which Were being reworked by the crafts. Like welders, 10 they are coming back and stripping coatings off pipe hangers.

11 There should be some tracking of the coatings. These were 12 already bought off coatings that were being stripped and 33 they needed a tracking system to go back, identify coatings 14 which were now being stripped so that you would negate or 15 eliminate the final inspection reports where they were being bouhht off.

  • 18 system for this. The RFIC came back well over two months 39 later, and at this time a lot of these areas are now very, 20 very hard to get to. You know, they are up in the dome. They 21 have taken the catwalk of f out of the dome now. So, it makes l 1

sort of like an "if" situation. You don't know -- you know, 3

it's right on the very fine line -- whether or not you should 4 accept or reject.

5 Because of this pressure usually you know you are 6

going to get into all kinds of trouble if you are going to 7 reject that item. So, in that sense I felt like it had 8 quite a bit of effect on me. In my opinion, I think it is .

9 all calculated for this one purpose, to put a certain amount to of pressure on the inspector so that he will be reluctant 11 to write an "unsat" report.

12 O So, you can't really give me any specific instance 13 where this pressure that you felt inhibited you from doing 1 14 your job.

15 A You mean samples or persons?

16 O Did you ever write something " satisfactory" that 17 should have been "unsatisf actory" because someone told 18 you, "I'm going to break your legs. if you don't?"

19 A No, of course not.

20 0 Or even a little less than that.

21 A Well, I have written a " sat" report which(MT 22 tried to explain the intent of the procedure, which 23 I disagreed with. It was the intent -- he was trying to 24 explain but it was not actually in writing in the procedure.

25 He more or less told me to " sat" it out because he was the

2 A How you are supposed to do it, I don't know. How 3 I actually do it is by, you determine how much square 4 footage you have. You determine how many readings you are 5 supposed to take, and you take those readings. If it's 6 unacceptable, you define the area that's unacceptable and 7 you "unsat" it.

8 But this is giving you directions on how to 9 isolate an unacceptable area. It tells you how to 10 recalculate the average and to write those averages down.

11 I don't know how to do it by the procedure. I am doing it 12 my own way and as f ar as I know every other inspector is 13 doing it his own way.

14 Very few people know how to implement that.

15 0 You are saying no one basically understands the 16 intent of Note 4, then? ,

17 A Yes, sir.

18 Q Anything else?

19 A Well, this is back to 3.2.2-E, this note, plus 20 No. I here. There are several contradictions in there, as 21 we have already discussed, about five percent of every square 22 inch. There are numerous contradictions in there.

23 0 All right. I am looking for new information that 24 you have not given me already.

25 A Okay. One which draws a lot of confusion is

E meh 10 104 1 paragraph 3. 7. 5-B. It talks about, the last thing-it says, 2 " Pin holes, small discontinuities, may be repaired at the 3 time of final inspection without a later reinspection of the 4 repai r. "

5 The problem being is -- which I discussed with 6 Harry Williams -- is that you still have a criteria, a 7 continuity inspection criteria, on 3.6.4. You have an 8 , attachment on the back of this procedure which -- I think 9 it's called a NACE Condition C, a liable force of discontinuity 10 You have a criteria for determining how many 11 points discontinuity you are allowed within the area. But 12 here you are more or less supposed to ' disregard this, this 13 criteria, and use 3. 7.5-B which says, "Small discontinuities 14 can be repaired without a later reinspection of the repair."

15 So, on the one hand it should be "unsat" if you 16 go beyond that criteria. But you are supposed to assume

17 that it's automatically " sat"' because they have put this into l

! 18 the procedure, allowing you to touch up discontinuities.

21 A Yes. I would like to go to Construction Procedure 22 CCP 30.

23 This paragraph 4.4. 3 is ex'tremely confusing. I am l 24 not exactly sure what this deals with.

We have been talking, 25 you know, this af ternoon, concerning the coating interface as

meh 11 1 repair areas. But what this actually deals with is not 2 repair areas. This is talking about equipment.where there 3 is an interface between steel and concrete, like imbeds.

4 That is what we are talking about here, where if you have 5 imbed in concrete you can overlap the 305 and go'still on 6 to the 1201 on the concrete an inch and-a-half; that is what 7 they are dealing with. At least that is what we think they 8 are talking about.

g Q Have you written an RFIC on this question?

10 A Yes, sir, that is what I showed you earlier.

11 Q Oh, we have looked at that already?

12 A Yes, sir.

13 Q Okay. I lost track, I am sorry. When I go back 14 about three or four days from now and all this settles in,

. 15 maybe I will be able to find that.

16 A Yes. But this doesn't really concern a repair area 17 as far as CZ-ll or 305. This is talking about an interface 18 between steel and concrete.

19 The reason, you know, it deals with steel and 20 concrete is because if you go to CCP 40 which we found out, 21 it says that all coating interfaces shall be inspected during n the steel inspection. CCP 40 is the concrete procedure.

23 They are saying that whenever you come into an 24 interface with steel on the concrete, you don't do an inspectio n 25 at that point. You wait until you are actually performing an

inspection for steel. This is the paragraph you go to, and 2 apparently this is saying that you are allowed to apply 3 a finish coat for the steel over onto the finish coat for the 4 concrete. That is our interpretation.

5 But if you read that, it doesn't necessarily say 6 that.

7 o Well, where is the section that discusses repair 8 interfaces?

9 A This is it right here, we think.

10 Q But that is really the one, you said, applies --

11 does it apply to everything, maybe?

12 A APparently~so.

13 0 okay, but already you told us that you are 14 uncomfortable with that as applied to repair. You don't have

20 that because what you are doing is saying their work is 21 not satisf actory. l 22 So, they have a certain amount of right to ask you 23 what is wrong. But you know, sometimes they will tell you, 24 "You are wrong. I think you are wrong" and they go running I 25 off to the general foreman. The general foreman comes and l 1

6 A As far as inspector, I don't mind the going back and 7 forth and arguing about it, I don't consider that harassment, 8 intimidation, as long as it's kept right there and doesn't 9 get out of hand, and that is what happens at Osunche Peak, 10 it sort of gets out of hand and you need some support from 11 your supervisors to back you up and talk with the crafts 12 people and say, " Hey, you need to tell your guys to hold it 13 down because you are kind of interfering with their 14 inspection."

15 0 And you are saying basically that you are not getting lis the support you feel you should get from your supervision,-

18 A Exactly. We have never, ever gotten any kind of gg support from them in that regard.

6 A Yes, it's just your regular, on-going inspection 7 where, as part of your inspection attributes check the air 8 to make sure it doesn't have any oil or grease, or water e coming out of the air lines.

10 0 Okay. I guess also I am curious too, would you 11 not necessarily see, as far as any contaminants, couldn't 12 you see that? You do do tack tests to determine if it is 13 dry, right, before you apply your primer? j 14 BY MR. HAWKINS:

15 0 Do you use a blotter, is that what you are doing?

16 A Yes, just a blotter test on the air.

17 0 Is that what you are referring to?

18 A Yes, as far as air acceptability. But as far as 19 tack test, determining whether or not you are receiving a full M cure, no. That is my point, that you are not going to l 1

21 determine if it is uncured. The water is going to be in 22 minute amounts, it's not like a gallon of water coming through  !

23 the air hose.

24 But it will affect the curing of that coating 25 to a certain degree. It is going to restrict -- you won't l

receive an optimum level of molecular weight for' that polytter.

2 It will not react or polarize and achieve the molecular weight 3 it should get.

4 So, it could be ten years; it could be twenty 5 years, whatever. It is not going to have the optimum 6

physical properties it should if a certain amount of water, 7

even a limited amount of water, came through those air hoses.

8 That was what I was trying to point out to 9

. hat you will never gain the optimum physical 10 properties if that is allowed to happen.

11 MR. JOHNSON: I don't have any more.

12 MR. HAWKINS: All right, let's move on to the last 13 issue in Statement 15. In a letter here to 14 you state that, " Enclosed is another example of unvarifiable 15 coating application methods similar to those already 16 provided to you in my letter dated October 11, 1983.

17 "This request for informational clarification is 18 answered by the OE for coatings at CPSES, This 19 RFIC instructs QC inspectors to allow painters to apply 20 coatings without regard to the DBA qualified sequence of 21 coatings for the system. This practice is widespread at 22 CPSES as already shown in my previous letter."
