SUBJECT:

INSPECTION RESULTS, LAWRENCE LIVERMORE NATIONAL 4

LABORATORY PERSONNEL EXAMINATION OF RACEWAY SUPPORT BOLTED CONNECTIONS AT DIABLO CANYON (DN 50-275 AND 50-323)

The purpose of this memorandum is to document the results of an examination of bolted connections of raceway supports at Diablo Canyon. This effort was conducted for two reasons; first, we have received allegations related to the l

adequcy of the anchor bolts which secure raceway supports to concerte (RV-83-A-033), and second, based on the feed-back I have been receiving from fL the Lawrence Livermore National Laboratory (contract) personnel, it appeared that adequacy of bolting of raceway supports might be a safety issue.

I requested the contract inspectors to tabulate and transmit to me the number of i

raceway supports examined, the number of bolts / nuts examined, and the number and type of deficiencies observed. This information was transmitted to me on November 8, 1983 and is current as of October 28, 1983.

One was found loose, licensee tightened and documented with a MVR Lack of Thread Engagement ---- Several Thousand examined:

One lacked proper engagement, licensee corrected and documented with a MVR Based on this information, I recommend we continue to use our contract inspectors to examine raceway supports at the current level of effort. Should the licensee's deficiency rate decrease further however, I think we should examine some other area with potential problems.
