r NOTICE OF VIOLATION Tennessee Valley Authority Docket Nos. 50-327 and 50-328

Sequoyah Units 1 and 2 License Nos. DPR-77 and DPR-79
]
The following violation was identified during the equipment qualification inspections on January 6-17, February 10-14, and June 23-27, 1986. The j
severity level wiss assigned in accordance with NRC Enforcement Policy (10 CFR 2 Appendix C). References are to the portions of the applicable l
inspection reports which describe the details of the deficiencies.
j Paragraph (f) of 10 CFR 50.49 requires that "Eoch item of electric equipment important to safety must be qualified by one of the following methods.
(1) Testing an identical item of equipment under identical conditions or under similar conditions with a supporting analysis to show that the j
equipment to be qualified is acceptable.
(2) Testing a similar item of equipment with a supporting analysis to show that the equipment to be qualified is acceptable.
(3) Experience with identical or similar equipment under similar conditions with a supporting analysis to show that the equipment to be qualified is I
acceptable.
(4) Analysisincombinationwithpartialtygetestdatathatsupportsthe analytical assumptions and conclusions.
Contrdry to the above Tennessee Valley Authority (TVA) failed to adequately establish qualification of certain electric equipment important to safety.
Instances where TVA failed to adequately establish qualification of electric j
equipment. important to safety are provided below.
1.
TVA had not adequately established qualification of Essex power and control electric cable in that the cable's ability to meet its required functional performance require.nents was not demonstrated since qualifi-cation test instrumentation data was not available.

2.
TVA had not adequately established qualification of ASCO solenoid valve model 206-381 in that the valve's ability to function during its required accident and/or post-accident environment of submergence was not demon-strated by acceptable testing.

TVA had not adequately established qualification for Nutherm 32KW air duct heater and control components in that similarity between tested andinstalledequipmentwasnotestablishedandreguiredpost-accident operability was not demonstrated.

TVA had not adequately established qualification for GA Technologies RD-23 radiation detector and its associated Brand-Rex coaxial cable in that the detector's and cable's ubility to meet their required function performance requirements was not demonstrated.

TVA had not adequately established qualification for Eaton multsconductor signal cable, in that the cable's ability to meet its required functional performance requirements relative to system loop accuracies was not demonstrated.

6.
TVA had not adequately established qualification for Limitorque motor valve operators for their installed condition without T-drains in their limit / torque switch compartments.
(Paragraph 4.0.(1)(h),50-327/86-01-13; 50-328/86-01-13).

TVA had not adequately established qualification of Barton Lot 7-764 trans-mitters 2-LT-3-38, -43, and -56 in that they or their associated conduit connections were installed in locations inside Unit 2 which would subject them to accident conditions of flooding and they w)ere not qualified for submergence.

TVA had not adequately established qualification of Raychem splices (including breakouts and motor connection kits) in that splices installed in the plant were different from qualified configurations.

This is a Severity Level IV Violation (Supplement I).
During inspections on January 6-17, February 10-14, June 23-27, and December 8-12, 1986, and April 6-10, 1987, the NRC reviewed TVA's corrective actions relative to the instances cited in this Notice of Violation and found the actions to be acceptable. Since TVA's corrective actions are considered adequate, response I
pursuant to 10 CFR 2.201 for this Notice of Violation is not required.
