This special report is being subnitted pursuant to the require-ments of Technical Specification 3.7.10 Action Statement (a) to inform you of two Fire Rated Assemblies being inoperable in excess of seven days.
Fire Door #227 (Unit 1, Access Door to the Turbine Driven Auxiliary Feed Pump Room) has been inoperable since August 26, 1986 due to the door closer coordinator's failure to operate as required. A design change request has been initiated to modify the door fre.me in the area of the door closer coordinator. This modification is necessary to strengthen the frame so as to prevent the door closer coordinator from tilting down into the path of the closing doors and obstructing them. Until the time that the design change review process is completed and implementation of repairs is achieved, Technical Specification 3.7.10 Action Statement requirements will continue to be met while the fire door is in an inoperable condition.
Fire Door #350 (Unit 2, Access Door in the 4KV Electrical Switchgear Room) was inoperable between September 12, 1986 and September 22, 1986 due to failure on the part of the responsible supervisor to take appropriate action within the required time. The failure of this door to be repaired in a prompt and timely manner was due to the supervisor responsible for the work not properly identifying the job's requirements when it was issued. After the supervisor recognized the significance of the door in question, repairs were implemented to return the door to operable status. The fire door was subsequently repaired and returned to service on September 22, 1986. Emphasis on jobs requiring immediate attention has been addressed to all concerned parties.
