Inspection Report No. 50-293/86-38
This refers to the special safety inspection (50-293/86-38) conducted by
Dr. M. McBride and others of this office on November 12 - 14, 1986, at the
Pilgrim Nuclear Power Station, Plymouth, Massachusetts. Circumstances sur-
rounding the isolation of the fire water system were reviewed. The findings
were discussed with Mr. A. Pederson of your staff.

The results of this inspection are described in the enclosed inspection report.
One violation of NRC requirements regarding the adequacy of fire system draw-
ings was identified. This violation is described in the attached Notice of
Violation. You are required to respond to this violation following the direc-
tions contained in the Notice.
From the findings of this inspection, it appears that weaknesses in the Pilgrim
fire protection program continue to exist.
In this instance, an inaccurate
drawing resulted in the primary fire water supply system for the station being
rendered inoperable for about six days. However, during the inspection we
found that weaknesses in fire brigade training and the unavailability of other
fire equipment due to outstanding maintenance activities combined with the
inoperability of the fire water supply system to substantially degrade your
onsite fire fighting capability. Although we noted that backup support from
the offsite fire department remained available throughout this period, we are
concerned that reliance on these backup sources instead of the normal onsite
system would result in a less timely response to a significant fire onboard the
station. Consequently, in your response to the violation, you should provide a
description of those actions planned or taken to improve the management atten-
tion and the priority afforded to fire system operations and maintenance and to
upgrade training for your onsite fire brigade members.
The responses directed by this letter and the accompanying Notice are not sub-
ject to the clearance procedures of the Office of Management and Budget as
required by the Paperwork Reduction Act of 1980, PL 96-511.
