INSERVICE INSPECTION - EXAMINATION

SUMMARY

PRAIRIE ISLAND NUCLEAR GENERATING PLANT - UNIT l' February 20, 1987 to March 3, 1987 j

1.0 INTRODUCTION

This report is a summary of the examinations performed during a forced outage at the Prairie Island Nuclear Generating Plant - Unit

1. The examinations were performed during the plant's forced out-age from February 20, 1987 to March 3, 1987. Prairie Island - Unit

. I began commercial operation on December 16, 1973.

This report identifies the components examined, the examination methods used, the examination number; and, summarizes the examina-tion results of each of the following areas:

1. Pressure retaining components and supports of the reactor coolant and associated systems classified as ASME Class 1 and 2.
2. Eddy current examination of the Steam Generator Tubing.

2.0

SUMMARY

OF RESULTS The evaluation of all the results from the inservice examinations indicated that the integrity of these systems has been maintained.

To assure continued integrity of the steam generators (S.G.), a to-tal of 19 tubes in S.G.12 were mechanically plugged.

3.0 ASME CLASS 1 3.1 EXAMINATION PLAN The examination plan focused on the pressure retaining compo-nents and their supports of the reactor coolant and associated auxiliary systems classified as ASME Class 1.

The examination plan was based on the examination requirements of the ASME Boiler and Pressure Vessel Code Section XI, 1980 Edition through and including the Winter 1981 Addenda, and complied with Prairie Island's Technical Specification, Sec-tion TS 4.15. The examination is in accordance with the pro-gram submitted to the United States Nuclear Regulatory Commis-sion on March 27, 1981 titled, "ASME Code Section XI Inservice Inspection and Testing Program and Information Required for NRC Review of Requests for Relief From ASME Code Section XI Requirements".

3.2 EXAMINATION METHODS Ultrasonic examination methods and techniques were used to 5 perform the volumetric examinations. The ultrasonic test sys- g tems consisted of an ultrasonic digital / analog tester and a a:

two channel strip chart recorder. One channel of the recorder Of was calibrated to reflect ultrasonic screen height (amplitude) $

and the second channel was calibrated to indicate metal path g a.

,I (range) to the reflector. This approach to the examination gives a permanent record to the extent possible.

j Liquid penetrant or magnetic particle examination methods were used to perform the surface examinations. The liquid pen-etrant examinations were performed using color contrast-solvent removable materials. Magnetic particle ex-aminations were performed using either a yoke with dry powder or an AC L-10 coil with fluorescent prepared bath.

All visual examinations were aided, when necessary, with arti-

ficial lighting and verified for adequacy with an 18% neutral gray card with a 1/32 inch black line.

3.3 EXAMINATION PROCEDURES The ultrasonic examination procedures for piping welds com-plied with the requirements of Appendix III of ASME Section XI that was issued in the Winter 1981 Addenda. All other proce-dures complied with the requirements of the 1980 Edition through and including the Winter 1981 Addenda of ASME Section XI. A listing of the procedures used for the examinations is shown in Table III of Appendix B.

3.4 E0VIPMENT AND MATERIALS All equipment and expendable materials used in the examina-

' tions are listed either by serial number or type along with their respective calibration date or batch number in Table IV of Appendix D. -

The ultrasonic calibration standards used in the examinations are listed in Table II of Appendix D. These standards are owned and maintained by NSP at the plant site.

3.5 PERSONNEL Northern States Power Company contracted Lambert, MacGill, Thomas, Inc. to perform balance of plant examinations. Hart-ford Steam Boiler Inspection and Insurance Company, represent-ing ANII, provided the Authorized Inspection.

All personnel involved in the performance or evaluation of ex-aminations are listed along with their title, organization and ASNT Level of Certification in Table I of Appendix 0.

Certifications for examination personnel are maintained on file by Northern States Power Company.

3.6 EVALUATION (a

I l The ultrasonic examiner (s) was aided in their evaluation by a calibration performed on a standard reference before each j

day's examination, checked before and after each individual examination and at intervals not exceeding four (4) hours. In addition, the ultrasonic data was recoroed on strip charts which were made a part of the inspection report, and permitted further evaluation.

3.7 EXAMINATION REPORTS AND DOCUMENTATION All examination reports and documentation are maintained on file by Northern States Power Company. Table I of Appendix A, B, and C identifies the examination report number (s) for each item examined. Many of the items identify more than one ex-amination report because of the different types of examina-tions performed on the item.

Table I of Appendix A, B, and C summarizes all the examina-tions performed to date and identifies the amount that will be examined in the future to complete the ten year examination requirements. For retrieval purposes, the prefix of the in-spection report number corresponds with the year the inspec-tion was performed. The examination report numbers for this outage are prefixed with "87 ".

Table II of Appendix A, B, and C compares the baseline ex-amination or previous results with the results obtained during this examination. Table III of Appendix A, B, and C identi-fies the isometric drawings that were used for the examinations. The personnel, ultrasonic calibration blocks, procedures, equipment and materials that were used for the in-spection are identified in the tables of Appendix D. Appendix G contains the Form NIS-1 titled, "0wner's Data Report for Inservice Inspections".

I The anomalies were either accepted by an engineering evalua-tion or acceptable per code; the hanger that was found to be non-functioning was turned over to the system engineer and a design modification was issued; the rounded PT indications were found acceptable per IWB 3514.

4.0 EXAMINATION OF THE STEAM GENERATOR TUBING During the February 1987 unscheduled leaker outage 100% of all ac-cessible tubes in steam generator 12 hot leg and 23% of the acces-

. sible tubes in steam generator 12 cold leg were examined. Included in the cold leg examination were 40 tubes of special interest (pre-viously degraded tubes). The examination was conducted utilizing the multifrequency eddy current technique. The probes were all differentially wound bobbin coil design multiplexed at frequencies of 400KHZ, 200KHZ, 100KHZ and 10KHZ in both the differential and absolute modes. The program was as follows:

1. Cold leg examinations were performed from the seventh support plate on the hot leg side through the tube end on the cold leg side. These examinations were completed using 0.720 inch diam-eter magnetically biased probes.
2. Hot leg examinations were performed from the first support plate on the hot leg side through the tube end on the hot leg side. These examinations were completed using 0.740 inch diam-eter magnetically biased probes.
3. A select sample (approximately 78) of tubes were examined with the rotating pancake coil (RPC) technique in steam generator 12 hot leg. The inspection strategy was to supplement the bobbin coil data and further characterize indications of percent through wall, undefined and distorted signals. Included were 31 tubes which were examined with the RPC technique in March 1986.

Westinghouse Electric Corporation was contracted to acquire and evaluate the eddy current data. Canam Inspection was contracted to perform a completely independent evaluation of all data acquired.

The scope of all the work contracted was completed using remote po-sitioning devices and the Zetec MIZ-18 digital test equipment along with associated acquisition and DDA-4 analysis software.

Summaries of tubes examined, indication distribution, tubes plugged this outage and total tubes plugged can be found in Tables I through IV respectively.

Tube sheet maps and cumulative lists of plugged tubes and indica-tions by depth range can be found in Appendices E and F for steam generator 12 hot leg and cold leg respectively. @

9.) Inspection Interval: 12-16-83 to 12-16-93 10.) Abstract of Examinations: Include a list of examination and a statement concerning status of work required for current interval.

This is a summary of the inspections conducted during a force outage of period one of the 2nd Ten Year Inspection Interval. The examinations were conducted on systems classified as ASME Class I and II. Eddy Cur-rent examination requirements for steam generator tubes was conducted this outage in accordance with Prairie Island Technical Specification Section TS 4.12.

11.) Abstract of Conditions Noted: Other than steam generator (S.G.) tubes, there were no signs of degra dation to systems scheduled for examina-tion. There were a total of 19 tubes for S.G. No. 12 were found ex-hibiting wall thinning greater than 45%.

The anomalies detected were: 4 rounded PT indications; I hanger with a non-functioning load settings.

12.) Abstract of Corrective Neasures Reconnended and Taken:

All degraded S.G. tubes with greater than 45% wall thinning were me-chanically plugged, a total of 19 tubes for S.G. No.12.

All anomalies were either accepted by an engineering evaluation or ac-ceptable per code. The hanger with the non-functioninf was turned over to the system engineer and a design modification was issued; the rounded PT indications were found to be acceptable per IWB 3514.
